摘 要:对国内外抗环瓜氨酸肽(CCP)抗体与系统性红斑狼疮(SLE)侵蚀性关节炎文献进行Meta分析。收集SLE临床病例进行研究。检索Medline、Pubmed、CNKI数据库、万方数据库,运用ReVMan5.2软件进行Meta分析。收集2012年~2014年辽宁医学院附属第一医院SLE病例。纳入8篇文献,病例组82例,对照组647例。CCP抗体在SLE侵蚀性关节炎中的OR值及95%CI分别为21.9和10.28~46.81。收集220例SLE病例,关节炎的发生率为52.7%(116/220)。关节炎组CCP抗体阳性率为22.0%(25/116),包括18例侵蚀性关节炎,阳性率为83.3%(15/18),非侵蚀性关节炎为10.2%(10/98)(P〈0.05)。因此,CCP抗体可作为区分SLE中侵蚀性关节炎与非侵蚀性关节炎的血清标志物之一。To systematically meta analyzed anti cyclic citrullinated peptide antibody in erosive arthritis of domestic and international patients with systemic lupus erythemmatosus (SLE). The authors collected clinical SIzE cases. The Medline database, Pubmed database, CNKI database, Wanfang datebase were retrieved. A meta analysis was conducted by RevMan 5.2 software. We collected the cases of SLE from the First Affiliated Hospital of Liaoning Medical University. Eight papers were included, involving 82 cases in the case group and 647 cases in the control group. The odd ratio and 95% confidence interval were 21.93 and (10.28-46.81), respectively. The cases of SLE we collected were 220 from year 2012 to year 2014 and the prevalence of arthritis was 52. 7% (116/220). The positive rate of anti-cyclic citrullinated peptide antibody in arthritis cases was 22. 0% (25/116). In arthritis cases, including 18 erosive arthritis cases, the positive rate of anti-cyclic citrullinated peptide antibody was 83.3%(15/18), while this rate was 10.2% (10/98) in non- erosive arthritis cases (P〈0.05). Therefore, anti-cyclic citrullinated peptide antibody could serve as a serum marker of erosive arthritis in patients with SLE.
