援塞医疗队埃博拉病毒病的临床管理初探

摘　　要：目的探讨中国人民解放军援塞医疗队对埃博拉病毒病(Ebola virus disease,EVD)的临床管理。方法通过创建中方主导、塞方配合的国际合作医疗管理模式,实行人员规范化培训上岗、制度化管理,医院感染防控专家负责制,EVD临床病例分类隔离、分级治疗制,药品、卫生装备耗材分级、分区存放、计算机辅助申领、补充等管理制度。结果解放军医疗队队员与当地员工以1∶2.65的配置比例展开工作,制定了68类243条EVD临床管理及感染防控操作规程,规范化培训考核双方工作人员500余人次,收治EVD疑似患者773例(其中确诊患者285例,存活率51.23%),实现中心制度化运行,工作人员"零感染"、患者"零交叉感染"。结论医疗队在EVD留观与诊疗中心的工作人员管理、EVD病例管理、医院感染防控、医技保障等方面积累了有效的经验。Objective To explore the clinical managing stratagem of Chinese PLA medical aid team performed in Sierra Leone in Ebola virus diseases （EVD） outbreak. Methods Chinese PLA medical aid team has explored a series of China -Africa collaborative managing modal for EVD cases management which leaded by Chinese PLA staff and collaborated with local health care workers. Following management system were implemented： standard staff training and management, hospital infection prevention and control expert system, classified EVD case isolating and treating management, specialized IPC expert responsible system ensured the safety and validity of EVD case trea- ting procedure. High efficient pharmaceuticals and medical equipments provision system, computer assisted logistics materials application and supplement management Results Totally 3 batches with 115 professional in charge of EVD clinical treatment from the 302 military hospital of Beijing were deployed to Freetown. Based on the re -constructed China - Sierra Leone EVD holding ＆ treatment center, a proper international to local staff collocating rate as 1 ： 2.65 was set. Through establishing 68 categories with 243 clinical regulation items and standard operating procedure （SOP） of infection and control, over 500 staffs have been trained and evaluated strictly, 773 EVD suspected patients have been admitted and treated safely （285 of them were confirmed and survival rate was 51.23% ） from Sep 16, 2014 to March 18, 2015. Notably, Chinese PLA medical aid team has achieved the zero infection among its medical staff and zero cross infection among inpatients. Conclusion The knowledge we obtained through this outbreak, in- cluding staff collocation and management, EVD case management, infection prevention and control, and oversea logistical support should be very valuable references for the prevention and control of other outbreaks in the future.
