复方丹参滴丸联合阿司匹林治疗冠心病心绞痛的Meta分析  

Meta-analysis of Compound Danshen Dripping Pills with Aspirin in Treating Angina Pectoris of Coronary Heart Disease

摘　　要：目的：评价复方丹参滴丸联合阿司匹林治疗冠心病心绞痛的安全性和疗效。方法：检索1994~2015年期间CNKI、VIP、CBM、Wanfang、Pub Med数据库,收集复方丹参滴丸联合阿司匹林治疗冠心病心绞痛的随机对照临床研究,筛选符合纳入标准的文献,并应用改良Jadad表进行评价。采用Cochrane协作网免费提供的Rev Man 5.0专用软件进行Meta分析。结果：初始检索到文献共190篇,纳入符合标准27篇,Jadad表评价后得24篇,包含2275名患者,Meta分析结果显示,复方丹参滴丸联合阿司匹林治疗冠心病心绞痛较单用阿司匹林有明显优势。其中临床疗效：同质性检验χ2=6.61,df=18,P=0.99;合并效应值估计Z=10.46,P〈0.0001,OR合并=4.84,95%CI[3.60,6.51]。血脂水平改善：低密度蛋白（LDL）同质性检验χ2=425.77,df=7,P〈0.00001,合并效应值估计Z=2.71,P=0.006,MD合并=-0.77,95%CI[-1.32,-0.23]。结论：与单独服用阿司匹林相比,复方丹参滴丸联合阿司匹林治疗对临床疗效和血脂水平改善效果更好,不良反应少。Objective：To evaluate effectiveness and safety of compound danshen dripping pills with aspirin in treating angina pectoris of coronary heart disease. Methods ： Retrieving CNKI, VIP, CBM,Wan Fang medicine database, PubMed database, collecting randomized controlled trials Meta-analysis of compound danshen dripping pills with aspirin in treating angina pectoris of coronary heart disease, screening standard literature and evaluating the application by modified Jadad table. Using the cochrane collaboration free RevMan 5. 0 dedicated software for Meta-analysis. Results： Retrieving a total of 290 references and 27 included articles were in accordance with the accepting articles. The Jadad table evaluation after 24,included 2275 patients. Meta-analysis resuhs show that the compound danshen dropping pill with aspirin therapy of coronary heart disease angina pectoris aspirin alone has obvious advantages. The clinical curative effect： the homogeneity test chi-square = 6. 61, df = 18, P = 0. 99. Consolidation effect value estimate Z = 10. 46, P 〈 0. 0001 , OR merge = 4.84,95% Cl [ 3.60,6. 51 ]. Lipid levels improve ： low density protein （LDL） homogeneity test ehi-square = 425.77, df = 7, P〈0.00001. Consolidation effect value estimate Z = 2.71, P = 0.006, MD merger = 0.77, 95% CI [- 1.32, - 0.23 ] . Conclusion： Compared with aspirin alone, compound danshen dropping pill with aspirin therapy for clinical curative effect and blood lip- id levels improved effect is better, less adverse reaction.
