低温等离子消融和YAG激光辅助鼻内镜下鼻中隔矫正术治疗持续变应性鼻炎的比较研究

摘　　要：目的:分析低温等离子消融、YAG激光辅助鼻内镜下鼻中隔矫正术治疗持续变应性鼻炎的临床疗效差异。方法:选择120例接受住院治疗的持续变应性鼻炎患者,采用数字表法随机将其分为对照组和观察组,每组60例。对照组接受YAG激光辅助鼻内镜下鼻中隔矫正术治疗;观察组接受低温等离子消融辅助鼻内镜下鼻中隔矫正术治疗,比较两组患者的治疗效果、术后相关指标、血清免疫球蛋白E(Ig E)及白细胞介素4(IL-4)水平差异。结果:观察组显效率和有效率明显高于对照组,差异有统计学意义(x2=5.076,P<0.05);观察组VAS评分、水肿时间、脱痂时间、Ig E以及IL-4水平均显著低于对照组,差异有统计学意义(t=4.395,t=5.371,t=4.938,t=5.493,t=5.628;P<0.05)。结论:低温等离子消融辅助鼻内镜下鼻中隔矫正术有助于提高持续变应性鼻炎患者的治疗效果、可促进术后康复、降低血清Ig E及IL-4水平。Objective： To analyze the effect of low temperature plasma ablation, YAG laserassisted combine endoscopic nasal surgery on patients with continuing allergic rhinitis. Methods： Chosen patients with continuing allergic rhinitis as research subjects, randomly assigned to control group received YAG laser-assisted treatment combine endoscopic nasal surgery and observation group with low temperature plasma ablation assisted endoscopic nasal surgery,compared treatment effect, postoperative related indicators, serum Ig E and IL-4 levels differences. Results： Observation group efficiency, effectiveness（71.67%, 96.67%） was significantly higher than control group; VAS score（1.78＋0.64）, edema time（6.21±1.56）d, scab off time（7.24±1.83）d, Ig E（112.18±17.23）u/ml, IL-4（1.34±0.15）pg/ml levels were significantly lower than that control group（t=4.395, t=5.371, t=4.938, t=5.493, t=5.628; P〈0.05）. Conclusion： The low temperature plasma ablation assisted endoscopic nasal surgery can help improve the treatment of patients with allergic rhinitis continue to promote postoperative rehabilitation, reduce serum Ig E and IL-4 levels.
