出院阶段护理计划在冠心病患者中的应用

摘　　要：目的探讨出院阶段护理计划在冠心病患者中的应用。方法选择2013年1—6月在上海交通大学附属第六人民医院因冠心病住院的患者90例为对照组,选择2014年1—6月因冠心病住院的患者90例为观察组。对照组接受常规健康教育护理方式,观察组在对照组常规护理的基础上制订出院阶段护理计划,指导并监督患者的服药、休息、饮食和预防感染等方面。采用心境状态量表和治疗依从性评价表进行入组时和3个月后测评。结果入组时,两组患者心境状态中的7个维度比较差异无统计学意义(P>0.05);3个月后,两组在愤怒与混乱方面比较差异无统计学意义(P>0.05),在其他5个维度上比较差异有统计学意义(P<0.05)。3个月后,观察组在遵医用药、合理休息、按时复查、合理饮食、戒烟戒酒、合理锻炼和预防感染7个维度及总分评分上明显高于对照组,差异有统计学意义(P<0.01)。结论对冠心病患者在常规健康教育的基础上按计划实施出院阶段护理指导,能提高患者治疗的积极性,改善患者的心境状态,提高治疗的依从性,有利于改善患者的生活质量。Objective To investigate the application of nursing instruction in the discharge phase with coronary heart disease. Methods 90 patients with coronary heart disease in Shanghai Hospital, Shanghai group, and another in the observation in the patients Sixth People＇s Jiao Tong University hospitalized from January to June 2013 were enrolled in the control 90 patients with coronary heart disease hospitalized from January to June 2014 were enrolled group. The control group received traditional health education, and the observation group received nursing instruction in the discharge phase. The Profile of Mood States and Treatment Adherence Scale were used. Results Before intervention there were no significant differences in seven mood state dimensions between the two groups. Three months after intervention, there were significantly differences in five mood state dimensions （ P 〈 0.05 ） except anger and confusion. Three months after intervention, the status of medical compliance, reasonable rest, regular review, reasonable diet, abstaining from tobacco and alcohol, reasonable exercises, infection prevention and overall score in the observation group were significantly improved compared with the control group （ all P 〈 0.01 ）. Conclusion For the patients with coronary heart disease, nursing instruction in the discharge phase can significantly improve their enthusiasm of treatment, mood states, medical compliance and quality of life.
