香槟方对犬腹部手术后胃肠动力的影响

摘　　要：目的研究香槟方对腹部手术后胃肠功能紊乱的影响及可能作用机制。方法毕格犬行剖腹探查手术,在胃窦、幽门、十二指肠、空肠和结肠浆膜植入应力传感器记录犬胃肠运动,在胃体部安装胃瘘管供灌入香槟方,在颈外静脉内置慢性静脉插管供取血及注射药物。观察香槟方对术后消化间期(空腹)及消化期(进食)胃肠移行性复合运动(MMC)的影响,香槟方对剖腹探查术后血浆胃促生长素浓度的影响,阿托品及河豚毒素对香槟方的拮抗作用。结果手术组手术当日及术后1、2、3日犬胃窦、幽门、十二指肠、空肠、结肠的振幅指数,血浆胃促生长素浓度较对照组明显降低(P<0.05或P<0.01);而香槟方组犬各部位振幅指数、血浆胃促生长素浓度较剖腹探查手术组明显增加(P<0.05)。术后1、2、3日消化期犬香槟方给药后各部位振幅指数均较给药前明显增加(P<0.01)。犬各部位在香槟方给药后振幅指数较本部位术后明显升高(P<0.01),而静脉灌流阿托品或河豚毒素后,各部位振幅指数较本部位术后及香槟方给药后均明显降低(P<0.01)。结论香槟方能明显改善犬腹部术后胃肠动力减弱情况,其作用途径可能通过内源性胃促生长素释放及肠神经系统内胆碱能神经介导。Objective To study the effect and possible functional mechanism of Xiangbin Fang（香槟方） on gastrointestinal dysfunction after abdominal operation. Methods Exploratory laparotomy was performed on beagle.Stress sensors were implanted in antrum,pylorus,duodenum,jejunum and colon serosa to record gastrointestinal motility of dogs. Stomach fistula was installed in the gastric body for perfusing Xiangbin Fang. Chronic venous cannula was inserted in internal jugular vein for collecting blood and injecting medicine. The effect of Xiangbin Fang on gastrointestinal migrating motor complex（ MMC） during the period of interdigestive（ limosis） and digestion（ food taking） after operation,the effect of Xiangbin Fang on concentration of ghrelin in plasma after exploratory laparotomy,and the antagonistic effect of atropine and tetrodotoxin on Xiangbin Fang were observed. Results The amplitude index of antrum,pylorus,duodenum,jejunum and colon,and concentration of ghrelin in plasma of dogs in the operation group on the 0th,1st,2nd,and 3rd day after operation remarkably decreased compared with that in the control group（ P〈0. 05 or P〈0. 01）; The amplitude index in each part and concentration of ghrelin in plasma of dogs in the Xiangbin Fang group remarkably increased compared with that in the exploratory laparotomy group（ P〈0. 05）.During the period of digestion,the amplitude index in each part of dogs after giving medicine on the 1st,2nd,and3 rd day after operation remarkably increased compared with that before giving medicine（ P〈0. 01）. The amplitude index in each part of dogs remarkably increased after giving medicine compared with that after operation（ P〈0. 01）.The amplitude index in each part of dogs after perfusing atropine and tetrodotoxin intravenously remarkably decreased compared with that after operation and giving Xiangbin Fang（ P〈0. 01）. Conclusion Xiangbin Fang could remarkably improve gastrointestinal motility fading situation of dogs after abdominal operation. Its fun
