摘 要:目的 探讨1.5T MR多b值扩散加权成像(DWI)对前列腺增生(BPH)与前列腺癌(PCa)的应用价值.方法 收集经病理证实的BPH 48例和PCa 35例,分BPH组与PCa组对比研究.全部病例均采用飞利浦Intera 1.5T MR行多b值扫描,b值取600 s/mm2、1 000 s/mm2、1 500 s/mm2、2 000 s/mm2,对比分析DWI不同b值BPH组织、PCa组织与周围前列腺实质信号表现.结果BPH组:BPH组织与周围前列腺实质信号具有一致性,随b值增加信号逐渐减低,b值2 000 s/mm2信号显著减低.PCa组:肿瘤组织与邻近前列腺实质信号存在差异性,随b值增加肿瘤组织信号降低不明显,邻近前列腺实质信号减低;b值2 000 s/mm2邻近前列腺实质信号显著减低,与肿瘤组织信号对比显著.结论BPH与PCa鉴别,DWI最佳b值2 000 s/mm2.Objective To explore the value of multi-b-value diffusion-weighted imaging (DWI) in diagnosis of the benign prostatic hyperplasia (BPH) and prostate cancer (PCa) at 1.5T MR. Methods 48 patients with BPH and other 35 ones with PCa proved by pathology were regarded as BPH group and PCa one respectively. All patients underwent DWI {b=600,1 000,1 500 and 2 000 s/mm2 ) scanning with a 1.5T MR scanner. The signals among BPH,PCa and the surrounding prostate tissue with different b values were compared. Results In BPH group,the signals of the BPH and the surrounding prostate tissue were similar,and those were reduced slowly with increasing b values and with the lowest b value of 2 000 s/mm2. In PCa group, different signals were found between the tumor and the surrounding prostate tissue without reduced signal of tumor with increasing b value,but with reduced signal of surrounding pros- tate tissue. When the b value was 2 000 s/mm2 ,the signal of surrounding prostate tissue was obviously reduced,which was different from the tumor. Conclusion DWI with a b value of 2 000 s/mm2 can be used for differential diagnosis of BPH and PCa at 1. ST.
