摘 要:目的 探讨磁共振T2-mapping成像在髌软骨军事训练伤中的应用价值.方法 采用GE 1.5T磁共振对48例武警战士行常规及轴位T2-mapping扫描.对髌软骨进行ICRS分级,测量ICRS 0级髌软骨外侧、中间及内侧面髌软骨浅、中、深3层平均T2值;测量ICRS其他级别髌软骨损伤处T2值.结果 22例ICRS 0级髌软骨浅、中、深3层髌软骨平均T2值分别为(39.66±4.24) ms、(29.31±2.48) ms、(43.49±3.25) ms,呈从软骨浅层至深层先降低后升高趋势(F=377.159,P=0.00<0.01);外侧、中间、内侧面髌软骨平均T2值差异无统计学意义(F=0.528,P=0.59>0.05);ICRSⅠ、Ⅱ、Ⅲ级损伤平均T2值均高于对照组(t=23.13、22.69、18.82,P=0.00<0.01).损伤处T2值与ICRS分级呈正相关关系(r=0.821,P=0.00<0.01).结论 磁共振T2-mapping成像对髌软骨军事训练伤的早期诊断具有较高的临床应用价值.Objective To evaluate the value of T2-mapping in injured patellar cartilage related to the military training. Methods 48 soldiers from Chinese People's Armed Police Force were imaged both with a standard knee MRI protocol and with an axial multi- echo spin-echo T2 mapping. The T2 values of patelIar cartilage with ICRS 0 grade were measured at superficial,middle and deep zone of medial,middle and lateral surface of patellar cartilage. T2 values of injured patellar cartilage were measured. Results The mean T2 values of superficial,middle and deep zone of patellar cartilage with ICRS 0 grade in 22 patients were (39. 66±4.24) ms, (29. 31±2.48) ms,and (43.49±3.25) ms. Spatial differences in cartilage T2 values were statistically different (F= 377. 159, P =0.00〈0.01). There was no statistical difference between the mean T2 values of medial, middle and lateral surface of patellar cartilage (F=0. 528,P=0.59〉0.05 ). The mean T2 values in injured groups were higher than those in control group (t=23.13, 22.69, 18.82,P=0.00〈0.01). Spearman' s rho correlation analysis revealed a positive correlation between T2 values and ICRS grade ( r= 0. 821, P = 0.00〈0.01 ). Conclusion MRI T2-mapping might be a sensitive method for the detection of early patellar cartilage injury related to military training.
