摘 要:目的比较分析浙江省东部地区不同时期丙型肝炎病毒(HCV)基因型的分布,为制定相应的防治策略提供参考。方法选择2008年1月-2013年4月就诊的338例慢性HCV感染者,分为早期组(2008年1月-2010年12月)231例和后期组(2011年1月-2013年4月)107例,采用基因芯片法进行HCV基因分型检测。结果 338例标本中共检出8种亚型,其中1a型、1b型、2a型、3a型、3b型、6型、1b+2a混合型、其他混合型,分别占1.18%、63.31%、11.84%、5.62%、5.92%、6.21%、4.74%、1.18%;两组中均以1b型为主要基因型,分别占70.13%和48.60%,但后期组1b型的构成比下降;2a型为早期组的次要基因型占10.82%,而6型为后期组的次要基因型占14.95%;早期组1b+2a混合型的构成比高于后期组(χ2=4.651,P=0.047),而1a型和6型的构成比均低于后期组(χ2=8.423和17.41,P<0.001);男性患者在早期组混合型的构成比较高(χ2=6.270,P=0.012),而在后期组中为6型(χ2=13.553,P<0.001);各基因型的年龄构成差异均无统计学意义。结论浙江省东部地区HCV基因型同样以1b型为主,但随着时间的推移,HCV基因型分布产生较大变化,其变化可能与人口流动和社会活动的增加相关。OBJECTIVE To observe and compare the distribution of genotypes of hepatitis C virus (HCV) in eastern Zhejiang province in different periods so as to put forward corresponding prevention strategies .METHODS A total of 338 patients with chronic HCV infection who were treated in the hospital from Jan 2008 to Apr 2013 were en‐rolled in the study and divided into the early period group with 231 cases ( from Jan 2008 to Dec 2010 ) and the late period group with 107 cases ( from Jan 2011 to Apr 2013 ) .The genotypes of HCV were detected by using gene chip .RESULTS Totally 8 subtypes were detected in 338 specimens ,including the genotype 1a (1 .18% ) , genotype 1b (63 .31% ) ,genotype 2a (11 .84% ) ,genotype 3a (5 .62% ) ,genotype 3b (5 .92% ) ,genotype 6 (6 .21% ) ,genotype 1b+2a (4 .74% ) ,and other mixed genotypes (1 .18% ) .The genotype 1b was the most com‐mon genotype and accounted for 70 .13% in the early period group ,48 .60% in the late period group ;however , the constituent ratio of the genotype 1b was decreased .The genotype 2a was the secondary genotype in the early period group ,accounting for 10 .82% ,while the genotype 6 was the secondary genotype in the late period group , accounting for 14 .95% .The constituent ratio of the mixed genotype 1b+2a of the early period group was higher than that of the late period group (χ2 =4 .651 ,P=0 .047) ,however ,the constituent ratios of the genotypes 1a and genotype 6 of the early period group were lower than those of the late period group (χ2 =8 .423 and 17 .41 ,P〈0 .001) .The constituent ratio of the mixed genotype in the male patients was relatively high in early period group (χ2 =6 .270 ,P=0 .012) ,the genotype 6 in the late period group (χ2 = 13 .553 ,P〈 0 .001) ,and there was no significant difference in the constituent ratio of age among the genotypes .CONCLUSION The genotype 1b is domi‐nant among the genotypes of HCV in the eastern Zhejiang province ,however ,the distribution of the
