摘 要:目的探讨大鼠克雷伯杆菌肺炎模型的临床表现、C反应蛋白(CRP)、皮质醇水平及治疗后的变化。方法4周龄清洁级大鼠随机分为模型组、干预组、对照组,每小组8只。模型组、干预组接种相同浓度、剂量的肺炎克雷伯杆菌菌液,对照组接种0.9%氯化钠注射液。接种成功后24h,干预组给予头孢哌酮钠舒巴坦钠治疗。各小组抽样行x线检查,并分别于第3、5、7d取血检测CRP、皮质醇。结果第3d,模型组CRP、皮质醇升高,与对照组比较有显著性差异(P〈0.01),模型组CRP较干预组高;第5d,模型组CRP继续升高,而皮质醇则降低,与对照组、干预组有显著性差异(P〈0.01),干预组第5d CRP、皮质醇降至正常;第7d,模型组CRP继续升高,而皮质醇继续降低,与对照组、干预组有显著性差异(P〈0.01)。结论大鼠肺炎模型急性感染期CRP、皮质醇升高,与肺炎病理过程密切相关;用抗生素后下降,能有效控制炎症发展。CRP、皮质醇可作为炎症控制的监测指标。Objective To explore clinical manifestation of rats with klebsiella pneumonia and the level of C - re- action protein(CRP) as well as cortisol before and after treatment. Methods 4 weeks old clean rats were randomly divided into model group, control group and interference group. Each group was divided into three small groups with 8 rats in each small group. Rats in model group and interference group were inoculated klebsiella pneumoniae bacteria with the same concentration and dose. Rats in the control group were inoculated O. 9% sodium chloride injection. Rats in interference group were treated by cefoperazone sodium and sulbactam sodium 24 hours after inoculation. X - ray examination was taken in each group. CRP and cortisol were detected on the 3,5, 7 day. Results The levels of CRP and cortisol in model group increased on the third day. There was a significant difference between model group and control group(P 〈 0.01 ). The levels of CRP continued to rise, and cortisol decreased on the fifth day. There were significant differences between model group and control group as well as interference group(P 〈0. O1 ). The levels of CRP continued to rise, and cortisol continued to decrease on the seventh day. There were significant differences be- tween model group and control group as well as interference group ( P 〈 0.01 ). Conclusion The levels of CRP and cortisol are gradually increased in model group, which are closely related with pathological process of pneumonia. The levels of CRP and cortisol decrease after using antibiotics, which can control inflammation effectively. The levels of CRP and cortisol can be used as clinical indicators tor inflammation control.
