摘  要:目的探讨CT导引下经皮穿刺抽吸、无水酒精硬化治疗肾囊性病变的临床应用价值及技术要点。方法 30例患者治疗37个肾囊性病灶,其中单纯性肾囊肿23例,肾盂旁囊肿4例,多囊肾2例,肾囊性转移1例。在CT导引下,23例单纯囊肿及肾盂旁囊肿4例患者顺利完成穿刺无水酒精硬化治疗,1例多囊肾行囊肿减灶减压治疗,1例肾囊性转移患者行无水酒精+平阳霉素硬化治疗。结果 37个囊肿病灶术后追踪观察3个月至1年,单纯肾囊肿疗效为93%(25/27),其中囊腔消失为88%(24/27),2例多囊肾减灶减压治疗,腰痛症状消失。1例肾囊性转移囊性病灶消失。临床症状缓解率为87%(26/30),并发症为局部疼痛27例,血尿7例,无严重并发症出现。结论 CT导引经皮穿刺无水酒精治疗对单纯肾囊肿和囊性病灶是一种安全、并发症低、疗效高的治疗方法。Objective To investigate the clinical application value of CT guided percutaneous needle puncture aspiration and technical points, anhydrous alcohol sclerotherapy for renal cystic diseases. Methods Thirty seven renal cystic lesions were treated in 30 cases of patients, including 23 cases of simple renal cyst, 4 cases of parapelvic cyst, 2 cases of polycystic kidney and 1 case of renal cystic metastasis. Under the guidance of CT, 23 cases of simple cyst and renal cyst in 4 patients successfully completed the needle puncture aspiration and sclerosing treatment. One case of polycystic kidney underwent the treatment of cyst decompression. One case of focal reduction, renal cystic metastasis underwent the treatment of absolute alcohol sclerotherapy and Pingyang mycin. Results After a follow-up of 3 months to 1 years, the treatment effectiveness of simple renal cysts were 93% (25/27), where as the cystic cavity disappeared for 88% (24/27), by stoves decompression in the treatment back pain disappeared in two cases of polycystic kidney for one case of renal cystic metastatic cystic lesions disappeared. Clinical symptom relief rate was 87% (26/30), local pain occured as complications in 27 cases, 7 cases of hematuria, there were no serious complications. Conclusion CT guided percutaneous absolute alcohol treatment is a safe, less complications, and high curative value method in the treatment of simple renal cysts and cystic lesions.
