摘 要:目的了解接受化疗的急性白血病患者医院感染状况,为及时采取干预措施提供科学依据。方法采用前瞻性监测方法,对某三级教学医院血液科住院接受化疗的急性白血病患者医院感染进行目标性监测。结果共调查该医院血液科接受化疗的急性白血病患者994例,发生医院感染患者277例,感染率为27.87%。在化疗的诱导缓解期、巩固化疗期和难治复发期,感染率依次为50.42%、18.27%和29.45%。急性白血病患者医院感染部位以上呼吸道多见。277例感染的急性白血病患者送检病原学标本中,共检出病原菌442株,以革兰阳性菌占主导。结论接受化疗的急性白血病患者是医院感染发生的高危人群,在诱导缓解期感染率最高,应采取对应措施控制医院感染的发生。Objective To understand the status of nosocomial infection in patients with acute leukemia who received chemotherapy,and to provide scientific evidence for the intervention measures. Methods Prospective monitoring method was used to carry out the objective monitoring of nosocomial infection in patients with acute leukemia who received chemotherapy in the Department of Hematology in three grade teaching hospital. Results A total of 994 patients with acute leukemia who received chemotherapy in the hospital department of Hematology were investgated,277 patients had nosocomial infection,the infection rate was 27. 87%. The infection rate was 29. 45%,18. 27% and 50. 42% in the remission stage,the consolidation chemotherapy period and the refractory relapse stage respectively. Upper respiratory tract was the main location of nosocomial infection in patients with acute leukemia. 442 strains of pathogens were detected from 277 cases of infection in acute leukemia patients with submission of pathogenic samples,Gram positive bacteria were dominant. Conclusion Acute leukemia patients receiving chemotherapy is a high risk group in nosocomial infection,and the infection rate is the highest in the remission stage,and corresponding measures should be taken to control the occurrence of nosocomial infection.
