三级护理评估在精神科的实施和效果分析

摘　　要：目的：探讨三级护理评估在精神科的实施效果。方法选取2012年5月至2013年1月在我院精神科住院的精神分裂症急性发作期及器质性精神障碍患者106例,随机分为普通护理组50例和三级护理评估组56例。采用护士用住院患者观察量表（ Nurses’ Observation Scale for Inpatient Evaluation,NOSIE）对两组患者病情和疗效进行评定,3次评定时间：入院48 h、治疗2周与出院前。结果两组第1次评估,各项指标无显著差异（P〉0.05）。第2次和第3次评估,两组各指标有不同程度的改善（P〈0.05）,与第1次评估比较,第2次评估除总消极,两组指标明显改善（P〈0.05）,第3次评估两组指标明显改善（P〈0.05）。第2次评估中,与对照组比较,三级护理评估除社会能力和总消极无显著差异,其余指标均明显改善（ P〈0.05）。第3次评估中,与对照组比较,三级护理评估组除社会能力、社会兴趣和抑郁无显著差异,其余指标均明显改善（ P〈0.05）。与对照组比较,三级护理组的入院出院护理、健康教育、专业素质和责任心方面的评分较高（ P〈0.05）。结论精神科实行三级护理评估方法,发现患者的护理问题,采取预见性干预护理措施,可有效降低各类风险发生率,促进患者早日康复。Objective To explore the effect of＂ Three-level Nursing Evaluation＂ in psychiatric department. Methods From June 2012 to January 2013 , 106 cases of in-hospitalized patients with schizophrenia were randomly divided into two groups：50 cases of control group and 56 cases of three-level nursing evaluation group. Nurses＇ Observation Scale for Inpatient Evaluation （NOSIE） was applied to assess the patient＇s condition for 3 times： patients admitted to hospital within 48 hours, 2 weeks after treatment and before discharge. Results On the first assessment, all indicators of two groups had no significant differences （P〉0. 05） . On the second and third time, many indicators were improved （P〈0. 05） . Compared with the first assessment, all the indicators, except for the total negative second evaluation, were significantly improved （P〈0. 05） in the second and all the indicators in the third assessment were improved （P〈0. 05） . In the second evaluation, compared with the control group, except for social skill and total negative, the remaining indicators in three-level nursing evaluation group were significantly improved （ P〈0. 05 ） . In the third evaluation, compared with the control group, except for social skill, social interest and depression, the remaining indicators in three-level nursing evaluation group were significantly improved （P〈0. 05） . Compared with the control group, the indexes of hospital care and discharge care, health education, professionalism and accountability were higher （P〈0. 05） . Conclusion ＂ Three-level Nursing Evaluation＂ in psychiatric nursing can identify patients ＇ existing and potential care problems, appropriate care can be applied as soon as possible, which can effectively reduce various types of risk incidence and promote the recovery of patients.
