多发性肌炎/皮肌炎患者卵泡抑素样蛋白l及颗粒蛋白前体的水平及意义

摘　　要：检测多发性肌炎(polymyositis,PM)和皮肌炎(Dermatomyositis,DM)患者血清中卵泡抑素样蛋白l(follostatin-like-1,FSTLl)及颗粒蛋白前体(progranulin,PGRN)的水平,探讨其与PM和DM患者疾病活动度的相关性。采用ELISA检测36例PM和DM患者及25名健康对照组血清FSTLI及PGRN水平,并通过Mann.Whitney U检验、t检验及Pearson、Spearman相关分析检验进行统计分析。PM和DM组FSTLI/PGRN血清水平均明显高于健康对照组,与ESR、CRP均成一定程度的正相关,且存在肺间质纤维化患者FSTLI/PGRN水平明显增高,总疾病活动度评分与FSTLI/PGRN血清的浓度呈正相关;患者治疗后血清FSTLI/PGRN浓度与疾病活动度评分均显著下降,两者变化一致。FSTLl及PGRN在PM和DM患者血清中明显升高,且与疾病活动度评分相平行,提示FSTLl及PGRN可能反应病情的活动度,可作为PM/DM疾病活动的评价指标。This study aims to detect the levels of serum Follostatin-like-1 （FSTL1） and Progranulin （PGRN）, and to explore the relationship of FSTL1 and PGRN to disease activity in patients with polymyositis/dermatomyositis（PM/DM）. The levels of se- rum FSTL1 and PGRN in 36 cases of PM/DM patients and 25 healthy controls were tested by using enzyme-linked immunosor- bent assay （ELISA）, and the differences between the patient group and the normal controls were analyzed by Mann. Whitney U-test, t-test, Pearson-test and Spearman-test. The serum levels of FSTL1/PGRN in the PM/DM group were significantly higher than those of the healthy controls, and the values of ESR and CRP were to some degree positively correlated with serum FSTL1/PGRN concentrations, and the FSTL1/PGRN levels of patients with pulmonary fibrosis increased obviously, and the total disease activity scores were positively correlated with serum FSTL1/PGRN concentrations; both the serum FSTLI/PGRN concentrations and disease activity scores significantly declined after treatment. The levels of FSTL1 and PGRN were signifi- cantly increased in PM/DM patients, and were in parallel with disease activity score, suggesting that FSTL1 and PGRN may re- flect disease activity, so could be used as an evaluation indicator of PM/DM disease activity.
