经伤椎椎弓根植骨及短钉固定治疗胸腰椎骨折的临床研究

摘　　要：目的 探讨经伤椎椎弓根植骨及短钉固定治疗胸腰椎骨折的临床疗效。方法 将2008年11月~2014年5月我院收住的66例胸腰椎骨折患者分别采用经伤椎椎弓根植骨及短钉固定（A组）与传统跨伤椎上下椎4钉固定（B组）治疗,A组30例,B组36例,比较两组术前、术后3 d及末次随访时Cobb角、伤椎前缘高度比值和视觉模拟疼痛评分（VAS）。结果 末次随访时,A组VAS评分为（2.2±0.5）分,显著低于B组的（1.2±0.3）分,术后3 d及末次随访时,A组伤椎后凸角度矫正角度椎后凸角度矫正角度分别为（21.8±3.6）°、（23.8±2.4）°,均大于B组的（16.5±2.1）°和（17.8±3.8）°,术后3 d及末次随访时,A组伤椎前后缘高度分别为（5.3±1.5）%和（8.8±2.8）%。均大于B组的（2.2±1.2）%和（3.2±2.1）%。结论 与传统跨伤椎上下椎固定比较,经伤椎椎弓根植骨及短钉固定治疗胸腰椎骨折能有效恢复并维持伤椎高度及稳定性,疗效较好。Objective To explore the clinical effect of bone graft through pedicle of injury vertebra arch and short nail fixed in treating thoracolumbar fracture. Methods 66 patients with thoracolumbar fracture from November 2008 to May 2014 of our hospital were respectively treated with bone graft through pedicle of injury vertebra arch and short nail fixed （group A） and traditional across vertebral injury vertebral up and down 4 nail fixed （group B）.Group A was 30 cases and group B was 36 cases.Cobb angle,the ratio of injured vertebral leading edge height and visual analogue score （VAS） between two group before operation,after operation and the last follow-up were compared. Results At the last follow-up,VAS score in group A was （2.2±0.5）,significantly lower than （1.2±0.3） in group B.After treatment for three days and the last follow-up,injured vertebra back convex angle corrective angle and vertebra back convex angle corrective angle in group A was respectively（21.8±3.6） and（23.8±2.4）°,greater than（16.5±2.1）°and（17.8±3.8）°in group B respectively.After treatment for 3 days and the last follow-up,injured vertebral front and back leading edge height was （5.3±1.5）% and （8.8±2.8）% in group A respectively,significantly higher than （2.2±1.2）% and （3.2±2.1）% in group B respectively. Conclusion Comparing with traditional across vertebral injury vertebral up and down fixed,bone graft through pedicle of injury vertebra arch and short nail fixed in treating thoracolumbar fracture can effective recover and maintain the height of injured vertebral and stability,and can obtain good curative effect.
