摘 要:【目的】探讨自血光量子疗法对急性大面积脑梗死患者的美国国立卫生院卒中量表(NIHSS评分)、日常生活活动能力量表(BI评分)、血液流变学、血清神经元特异性烯醇化醇(neuron specific enolase,NSE)及S-100B蛋白的影响。【方法】选择86例经头颅CT或MRI确诊的急性脑梗死患者,随机分为常规治疗组(n=40)及光量子治疗组(n=46),分别于治疗前、治疗后5 d、12 d对患者进行NIHSS评分、BI评分及患者血液流变学、血清NSE及S-100B蛋白测定。【结果】光量子治疗组在治疗后5 d、12d NIHSS评分有显著改善(P<0.05)、BI评分较常规治疗组明显改善(P<0.05),血流变学指标好转、血液粘度降低(P<0.05),血清NSE及S-100B蛋白水平显著下降(P<0.05)。【结论】光量子治疗可以显著改善大面积脑梗死患者的血液流变学指标,降低血液粘度,降低血清NSE、S-100B蛋白水平,显著改善患者的神经功能缺损及日常生活能力,对急性脑梗死的神经元有保护作用。[Objective]To explore the effect of ultraviolet blood irradiation on NIHSS score, BI score, blood rheology, serum NSE and S- 100B protein in patients with acute massive cerebral infarction. [ Methods] A total of 86 cases with acute cerebral infarction diagnosed by head CT or MRI were selected and divided randomly into control group(n=40) and treatment group(n=46). Before the treatment, 5 d and 12 d after the treatment, the patients were evaluated by NIHSS score and BI score; blood rheology, serum NSE and S-100B protein were determined. [ Results ] Compared with the control group 5 d and 12 d after the treatment, NIHSS score, BI score and hemorheology indexes were obviously improved(P〈0.05); blood viscosity was reduced(P〈0.05); levels of serum NSE and S- IOOB protein decreased signifieantly(P〈0.05). [Conclusions] Ultraviolet blood irradiation can significantly improve hemorheology indexes in patients with massive cerebral infarction, reduce blood viscosity, decrease the levels of serum NSE and S- 100B protein, significantly improve the patients' neurological deficits and daily life ability and protect the neurons in acute cerebral infarction.
