摘  要:【目的】应用二维超声斑点追踪技术检测射血分数正常的重度主动脉瓣狭窄(aortic valve stenosis,AS)患者主动脉瓣置换术前后左心室纵向应变及应变率变化,并评价其临床应用价值。【方法】48例患者为主动脉瓣重度狭窄组(AS组),分别于术前及术后6个月测量常规超声心动图参数和左心室纵向应变、应变率。30例健康志愿者为对照组。【结果】AS组术前与对照组相比,左心室收缩末期内径、舒张末期内径及射血分数差异无统计学意义(P>0.05),但纵向应变、应变率均明显减低(P<0.01);术后AS组纵向应变、应变率较术前均明显增高(P<0.01),但低于正常对照组。【结论】二维斑点追踪技术能较常规超声更准确评价重度主动脉瓣狭窄患者瓣膜置换术前后左心室收缩功能,对于随访术后左室收缩功能的恢复也有重要临床价值。[Objective ] Using two-dimensional speckle tracking echocardiography, the changes in the left ventricular longitudinal strain (LS) and the longitudinal strain rate (LSR) were measured, in patients with aortic valve stenosis (AS)and preserved left ventricular ejection fraction (LVEF), before and after the aortic valve replacement (AVR). The clinical value of such measurements was discussed. [Methods] Forty-eight patients with AS underwent AVR were examined by conventional ultrasonography and two-dimensional speckle tracking (STE), their LS and LSR values were recorded 6 months pre and post AVR, respectively. The results were compared with those from 30 control cases. [ Results ] Compared with the control group, the left ventricular end systolic diameter (LVESD), end diastolic diameter (LVEDD), and EF values in AS patients had no significant difference before AVR (P〉 0.05). However, the LS and LSR values in AS patients were decreased significantly (P〈0.01). Six months after AVR, the LS and LSR values in the AS group were significantly increased (P〈0.01), although the values were still significantly lower than those in the control group (P〈0.05). [Conclusions] In AS patients undergone AVR, STE can evaluate left ventricnlar systolic functions more accurately than conventional ultrasonography. In the follow up study of recovery in left ventricular systolic functions, STE also has important clinical values.
