氯诺昔康与舒芬太尼用于口腔颌面外科手术后自控镇痛的临床疗效及安全性评价

摘　　要：目的评价氯诺昔康与舒芬太尼用于口腔颌面外科手术后静脉自控镇痛的临床疗效及安全性。方法将58例择期进行口腔颌面外科手术患者随机分为试验组28例和对照组30例。试验组予以氯诺昔康32 mg+格拉司琼3 mg+0.9%氯化钠至100 m L,接镇痛泵静脉自控镇痛;对照组予以1 mg·kg-1舒芬太尼+格拉司琼3 mg+0.9%氯化钠至100 m L,接镇痛泵静脉自控镇痛。比较2组患者术后4,8,12,24和48 h镇痛评分及镇静评分、满意度和不良反应发生率。结果 2组患者术后各时点镇痛评分及镇静评分差异无统计学意义(P>0.05)。2组患者术后镇痛满意度比较差异无统计学意义(P>0.05)。试验组的恶心呕吐及头晕等不良反应发生率为3.6%显著低于对照组23.3%(P<0.05)。结论氯诺昔康用于口腔颌面外科手术后自控镇痛效果与舒芬太尼相当,但不良反应发生率显著低于舒芬太尼。Objective To evaluate the clinical efficacy and safety of lor- noxicam or sulfentanyl on patient controUed intravenous analgesia （PCIA） in postoperative patients with oral and maxillofacial operation （OMS）. Methods Fifty -eight patients with oral and maxillofacial op- eration were randomly divided into treatment group （n = 28 ） and control group （n = 30）. Patients in the treatment group were given lornoxicam 32 mg ＋ granisetron 3 mg ＋ 0. 9% NaC1 up to 100 mL PCIA. And pa- tients in the control group were administrated of sulfentanyl 1 mg ~ kg-l ＋ granisetron 3 mg ＋ 0. 9% NaC1 up to 100 mL PCIA. After operation 4, 8, 12, 24 and 48 h, the pain control satisfaction and PCIA related adverse drug reactions such as nausea and vomiting and dizzy were evalu- ated between the two groups. Results The pain control, sedation effects and pain relief satisfaction had no statistical difference between the two groups in the above time points （P 〉 0. 05 ）. The PCIA related nausea and vomit or dizzy in treatment group was 3.6% which was significant lower than that in control group 23.3 % （P 〈 0. 05 ）. Conclusion Com- pared with sulfentanyl, Lornoxicam had the same pain control and seda- tion effects for PCIA in patients with oral and maxillofacial operation, but with less nausea and vomiting or dizzy adverse drug reactions.
