摘 要:目的研究骨折患者48 h内不同性别、不同年龄段骨转换标志物变化特征,为制定本地区骨转换标志物的参考值及研究骨质疏松症提供参考。方法收集2013年1月-2014年12月在本院住院的经影像学诊断为骨折患者(623例),入院后24 h内检测血清β-CTX、PINP、25-(OH)D3水平,按照性别、年龄进行分组,统计分析各年龄组的骨转换标志物水平及其变化特征。结果共有623例骨折患者,其中男性326例,女性297例,除61岁-65岁组PINP男、女组间差异无统计学意义(P〉0.05)外,其他各年龄组段男、女间各项目差异均有统计学意义(P〈0.01)。男性患者β-CTX、PINP随着年龄的增加有下降趋势,女性患者β-CTX、PINP随着年龄的增加有增高趋势,女性患者25-(OH)D3水平明显低于同年龄组男性。结论骨折后患者血清β-CTX、PINP男、女之间差异明显,不同性别的患者随着年龄变化,两者的变化趋势相反;25-(OH)D3与年龄无明显相关性。Objective To study the change characteristics of bone turnover markers in patients after fracture in different gender and age in 48 hours,so as to provide reference for setting the reference values of bone turnover markers in this region and for the study of osteoporosis. Methods 623 cases hospitalized patients diagnosed as fracture by imaging instrument were selected from January 2013 to December 2014. The value of β- CTX,PINP,25-( OH) D3 were tested within 24 hours after hospitalization.The patient was grouped according to sex and age,then,we analyzed the levels and variation characteristics of bone turnover markers in various age groups. Results There were 623 cases of bone fracture patients including 326 cases of male and 297 cases of female. There was statistical significance on the difference between man and woman in addition to the group of 61- 65 years old of the PINP( P〈 0. 05),and the differences on each project in all age groups were statistically significant( P〈 0. 01). With the increase of age,the value of serum β- CTX,PINP had downward trend in male patients and rising trend in female patients. The level of 25-( OH) D3 in female group was significantly lower than that of the same age group of male.Conclusion Serum bone turnover markers of PINP and β- CTX have significant difference between male and female with fracture. With the increase of age,bone turnover markers' change trend of different gender patients was opposite. There is no significant correlation between 25-( OH) D3 and age.
