Bakri球囊放置术在治疗产后出血36例临床分析

摘　　要：目的探讨Bakri球囊放置术治疗产后出血的临床效果。方法回顾性分析我院产科2010年1月至2013年12月84例产后出血患者的临床资料,分为Bakri球囊组36例,采用Bakri球囊填塞法止血,B-lynch组48例采用B-lynch缝合法进行止血。比较两组患者的止血效果结结局差异。结果术前Bakri球囊组与B-lynch组的出血量差异不显著(P>0.05),Bakri球囊组的术中出血量167.8±39.8(ml)、术后2h出血量44.6±10.5(ml)、术后24h出血量683.6±52.8(ml)、输血量2.5±0.6(U)、手术时间6.2±1.3(min)均显著低于B-lynch组(P<0.05);Bakri球囊组的24h血白蛋白98.7±6.2(g/L)显著高于B-lynch组(P<0.05)。Bakri球囊组的止血成功率77.78%显著的高于B-lynch组的54.17%(P<0.05),两组患者的子宫动脉结扎率、子宫切除率差异不显著(P>0.05)。结论 Bakri球囊放置术治疗产后出血较B-lynch缝合法能够更迅速的止血,止血效果更好。Objective： To investigate the clinical effect of Bakri balloon placement for treatment of postpartumhemorrhage. Methods： A retrospective analysis of our hospital from 2010 January to 2013 December in Department of obstetrics in84 cases of patients with postpartum hemorrhage clinical data,divided into Bakri group of 36 cases by Bakriballoon,balloon tamponade hemostasis,B- lynch group of 48 cases treated with B- lynch suture hemostasis. Comparison of the hemostasis effect of the two groups of patients with node outcome differences. Results： Preoperative bleeding Bakri balloon group and B- lynch group,but no significant difference（ P〈0. 05）,the amount of bleeding in operation of Bakri in balloon group 167.8 ＋ 39. 8（ ML）,2h after the operation,the amount of bleedingwas 44. 6 ± 0. 5（ ml）,24 h after the operation,the amount of bleeding was 683. 6 ± 52. 8（ ML）,blood transfusion amount is 2. 5 ± 0. 6（ U）,the operation timewas 6. 2 ± 1. 3（ min）were significantly lower than that in B- lynch group（ P〈0. 05）; Bakri balloon group 24 h serum albumin 98. 7 ± 6. 2（ g / L）was higher than that in B- lynch group（ P〈0. 05）. Bakri balloon group hemostatic success rate of 77. 78% was significantly higher than that in B- lynch group 54. 17%（ P〈0. 05）,uterine artery ligation in two groups of patients with uterine resection rate,no significant difference（ P〈0. 05）. Conclusio： Bakri balloon placement for the treatment of postpartum hemorrhage with B- lynch suture can be more rapidhemostasis,better hemostatic effect.
