摘  要:目的探讨糖化血红蛋白、同型半胱氨酸及尿微量白蛋白与2型糖尿病肾脏微血管病变的关系。方法选择95例2型糖尿病患者作为观察组研究对象,同期来医院参加体检的健康志愿者作为对照组,检测研究对象的糖化血红蛋白、同型半胱氨酸和尿微量白蛋白及血生化指标,分析比较各组糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平。结果糖尿病合并肾脏微血管病变组研究对象的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平高于单纯糖尿病组患者和对照组研究对象,同型半胱氨酸随血糖升高、肾脏损害程度的加深而升高,差异有统计学意义(P<0.05)。糖化血红蛋白水平升高,空腹血糖和尿微量白蛋白水平也随之增高。多因素Logistic回归分析显示糖化血红蛋白、同型半胱氨酸和尿微量白蛋白与糖尿病发生肾脏微血管病变呈正相关(OR值分别为1.27、1.41和1.53)。结论2型糖尿病合并肾脏微血管病变患者的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平显著升高,临床检测糖尿病患者的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白对预防糖尿病合并肾脏微血管病变有重要意义。Objective To investigate the relationship between glycosylated hemoglobin,homocysteine and urinary albumin and renal microvascular disease in type 2 diabetic nephropathy. Methods 95 patients with type 2 diabetes were selected as the study group of the observation group, the healthy volunteers who participated in physical examination during the same period served as the control group. Glycosylated hemoglobin, homocysteine, urine albumin and blood biochemical index of the subjects were measured, the levels of glycosylated hemoglobin, homocysteine and urinary albumin in the groups were compared. Results The levels of glycosylated hemoglobin, Hcy and Urinary albumin in diabetic patients with renal microvascular disease group were higher than those of the diabetic group and the control group. Homocysteine increased with the increase of blood glucose and renal damage degree,the difference was statistically significant (P〈0.05). The levels of glycosylated hemoglobin were elevated, and the levels of fasting blood glucose and urine albumin were also increased. Multivariate logistic regression analysis showed that glycated hemoglobin, with eysteine and urine albumin and diabetes occurrence of renal micro vascular lesions were positively related (OR = 1. 27, 1. 41 and 1.53). Conclusion The levels of glycosylated hemoglobin, homocysteine and urinary albumin were significantly increased in type 2 diabetic patients with renal microvascular disease, the clinical detection of glycosylated hemoglobin,homocysteine and urinary albumin in patients with diabetes mellitus is important for preventing diabetic renal microvascular disease.
