赵化南辨证用药促进自体动静脉内瘘成熟的疗效观察

摘　　要：目的:探讨全国名老中医赵化南辨证用药对血液透析患者动静脉内瘘成熟的促进作用。方法:将60例拟行动静脉内瘘手术的血液透析患者随机分为观察组和对照组各30例,对照组术前5 d开始用热毛巾热敷,术后第2天开始予喜疗妥涂擦,观察组术前5 d用益气活血通络方热敷,术后第2天开始采用清热解毒活血通络中药局部热敷。观察两组患者手术后1个月内动静脉内瘘通畅情况、内瘘初次使用时间及内瘘血液量。结果:观察组在动静脉内瘘通畅情况、内瘘初次使用时间、内瘘血液量等方面均优于对照组,两组比较有显著性差异(P<0.05)。结论:赵化南辨证用药局部对改善血液透析患者内瘘血流量,促进其发育成熟有较好的作用。Objective： To discuss the effects of hot compress with medicine based on syndrome differentiation by Zhao Hua-nan in promoting the maturity of the autogenous internM ＇arteriovenous fistula. Methods ： 60 cases with hematodialysis prepared to receive arteriovenous fistula surgery were randomized into observation group and control group, with 30 cases in each group. Control group were performed hot compress with hot towel 5 days before the surgery and applied with Hirudoid from the second day ＇after the surgery while observation group were performed hot compress with Qi-Benifiting, Blood-Activating and Collateral-Dredging Formula 5 days before the surgery and local hot compress of Chinese medicine with effects of clearing heat, removing toxicity, ac- tivating blood and dredging collaterals from the second day after the surgery. The obstruction improvement of the arteriovenous fis- tula, the first use time for internal fistula and the blood w,lume of the internal fistula in both groups one month after the surgel＇y were observed. Results： Observation group were better than control group in the aspects of obstruction improvement of the artefio- venous fistula, the first use time for internal fistula and the blood volume of the internal fistula; the difference was significant （P 〈 0. 05）. Conclusion： The local hot compress of medicine based on syndrome differentiation by Zhao Hua-nan can improve the blood flow volume of the patients with hematodialysis and promote the development and maturity of the fistula.
