健脾益肺口服液对慢性阻塞性肺疾病肺脾气虚证患者生存质量的影响

摘　　要：目的:观察健脾益肺口服液对稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)肺脾气虚证患者生存质量的影响。方法:将42例稳定期COPD,中医辨证为肺脾气虚证的患者随机分为治疗组和对照组。23例对照组患者采用常规西医治疗,19例治疗组患者在对照组基础上加服健脾益肺口服液,疗程均为1个月,疗程结束后继续观察6个月;观察两组患者疗效及治疗前后证候积分变化,BODE指数包括:体质量指数(body mass index,BMI)、第1秒用力肺活量(forced expiratory volume in first second,FEV1)、呼吸困难(mM RC评分)、6分钟步行距离(6-minute walk distance,6MWD);同时观察6个月内急性加重次数。结果:治疗组临床有效率95.65%,显著高于对照组84.21%,两组比较,差异有统计学意义(P<0.05)。两组患者治疗后证候积分均较治疗前降低,差异均有统计学意义(P<0.01)。治疗组治疗后6个月内患者的急性发作次数均明显少于对照组,两组间差异有统计学意义(P<0.05)。治疗组治疗后BMI、FEV1、mM RC评分及6MWD与治疗前比较,差异均有统计学意义(P<0.05)。治疗组治疗后BODE指数明显降低,与治疗前比较,差异有统计学意义(P<0.05)。对照组各项指标虽有改善,但治疗前后比较,差异无统计学意义(P>0.05)。结论:健脾益肺口服液治疗稳定期COPD患者疗效显著,可以显著改善患者呼吸困难、咳嗽等症状,同时提高机体免疫力,增加运动耐量,减少急性发作次数,从而改善患者生存质量。Objective： To observe the influence of JianpiY ifei liquid on the life quality of COPD patients in stable stage with Lung and spleen Qi deficiency syndrome. Methods： 42 COPD patients in stable stage with Lung and spleen Qi deficiency syndrome were randomly divided into two groups： control group（ 23 cases） received routine medical therapy,and treatment group（ 19 cases） was treated with JianpiY ifei liquid in addition to routine therapy. The course of treatment was one month and the two groups were observed in the following six months. The curative effect,TCM syndrome scores,BODE index[included body mass index（ BMI）,FEV1. 0 dyspnea（ mM RC scores）,exercise tolerance（ 6MWD） ] and the frequency of acute attack were observed throughout the course. Results： 1. The total curative effect（ 95. 65%） of treatment group was significantly better than the that of control group（ 84. 21%）（ P〈0. 05）. The TCM syndrome of both two groups got improved,with the scores markedly decreased（ P〈0. 01）. 2. The frequeny of acute attack was obviously lower than that of control group in the next 6 months with statistical difference（ P〈0. 05）. The parameters of BMI,FEV1. 0,mM RC scores,6MWD and BODE index of treatment group significantly decreased after treatment（ P〈0. 05）. There were improvement in above indexes in control group,but there were no statistical differences（ P〈0. 05）. Conclusion： JianpiY ifei liquid is effective in treatment of COPD patients in stable stage. It can markedly relieve patients＇ symptoms such as dyspnea、cough,increasing immunity function,enhancing exercise tolerance,decrease the frequeny of acute attack of COPD and improve the life quality of COPD patients.
