糖尿病足患者血清同型半胱氨酸及胱抑素C水平变化的研究

摘　　要：目的 探讨糖尿病足患者胱抑素C、同型半胱氨酸水平的变化及糖尿病足的相关危险因素.方法 选择自2012年1月至2014年7月在保定市第三中心医院内、外科住院的糖尿病患者134例,根据糖尿病足诊断标准将所有患者分为糖尿病组（63例）和糖尿病足组（71例）.分别测量HbA1c、肌酐、尿酸、血清胱抑素C、尿微量白蛋白及同型半胱氨酸水平,多因素Logistic回归分析糖尿病足的危险因素.结果 糖尿病足组同型半胱氨酸、胱抑素C及HbA1c均高于糖尿病组（t=2.37,-4.00,-2.96,P均＜0.05）.多因素Logistic回归分析显示同型半胱氨酸、胱抑素C、HbA1c、吸烟、体重指数为独立的危险因素,优势比分别为1.095 （95％ CI：1.031～1.162）、5.695（95％ CI：2.270～14.291）、1.807（95％ CI： 1.285～ 2.541）、13.401（95％ CI： 4.297～41.792）、1.246（95％ CI：1.027～1.510）,P均＜0.01.结论 糖尿病足患者同型半胱氨酸、胱抑素C、HbA1c水平升高.胱抑素C、同型半胱氨酸、HbA1c、吸烟是糖尿病足的独立危险因素.Objective To investigate the serum level of homocysteine and cystatin C in patients with diabetic foot,and the related risk factors of diabetic foot.Methods A total of 134 diabetic patients were enrolled in the Department of Internal Medicine and Surgery of the Third Center Hospital of Baoding City, from January 2012 to July 2014.All patients were divided into two groups according to the diagnostic criteria of diabetic foot：diabetic mellitus （DM） group （n =63） and diabetic foot （DF） group （n =71）.HbA1c, creatinine, uric acid, serum cystatin C, urine albumin and homocysteine were measured.The risk of diabetic foot was analyzed by Logistic regression.Results Serum homocysteine,cystatin C and HbA1c in DF group were higher than those in DM group,t value was 2.37,-4.00, and-2.96, respectively（all P 〈 O.05）.Logistic regression analysis of multiple factors showed that homocysteine, serum cystatin C, HbA1c, smoking, body mass index were independent risk factors, the odd ratio（OR） was 1.095 （95% CI： 1.031-1.162）, 5.695 （95 % CI： 2.270-14.291）, 1.807 （95 % CI： 1.285-2.541）, 13.401 （95 % CI：4.297-41.792）,1.246（95%CI：1.027-1.510）,respectively （all P〈0.01）.Conclusion Levels of cystain C and homocysteine are increased in patients with diabetic foot.Serum homocysteine, cystatin C, HbA1 c and smoking are independent risk factors for diabetic foot.
