摘  要:目的探讨富氢液对大鼠全脑缺血-再灌注损伤后X连锁凋亡蛋白抑制剂(XIAP)和第2个线粒体源胱天蛋白酶激活剂(Smac)的影响。方法成年雄性SD大鼠30只,随机均分为三组:假手术组(S组)、缺血-再灌注组(IR组)和富氢液组(H组)。采用经食管起搏诱发室颤建立大鼠全脑缺血模型。H组于再灌注即刻和6h时腹腔注射富氢液5ml/kg,IR组同时腹腔注射等容量生理盐水。再灌注24h行神经功能缺损评分(NDS评分)后处死,应用HE染色,光镜下观察海马CA1区病理学改变,计数海马CA1区锥体细胞,Western blot法检测脑组织中caspase-9,XIAP和Smac的蛋白表达,免疫组化法检测海马CA1区caspase-3的表达。结果与S组比较,IR组NDS评分明显降低,海马CA1区正常锥体细胞数明显减少(P<0.05),再灌注24hXIAP表达减少,Smac和caspase-9表达明显增加(P<0.05);与IR组比较,H组大鼠神经功能损伤明显减轻,海马CA1区正常锥体细胞数明显增加(P<0.05),再灌注24hXIAP表达增加,Smac和caspase-9表达明显减少(P<0.05)。结论富氢液能有效减轻大鼠全脑缺血-再灌注损伤,其机制可能是促进XIAP和抑制Smac表达,提高XIAP/Smac比值,从而发挥脑保护的作用。Objective To investigate effects of hydrogen-rich saline on the expressions of X-linked inhibitor of apoptosis protein (XIAP) and second mitochondia-derived activator caspase (Smac) following global cerebral ischemia-reperfusion injury in rats. Methods Thirty male Sprague Dawtey rats, weighting 280-320 g, were randomly divided into three groups(n= 10 each) :sham operation group(group S), ischemia- reperfusion group(group IR) ,hydrogen-rich saline group(group H). The method of ventricular fibrillation induced by transoesophageal cardiac pacing was used to produce global cerebral ischemia-reperfusion injury model. In group H, hydrogen-rich saline 5 ml/mg was injected intraperitoneally immediately and 6 h after reperfusion, respectively. The equal volume of normal saline was injected in group IR at the same time. Af- ter the neurological deficit score (NDS) was evaluated at 24 h of reperfusion, the rats were sacrificed. The brains were removed for microscpic examination of the area CA1 of the hippocampus by HE staining and de- termination of the number of normal pyramidal neurons. The expression of XIAP and Smac was determined by Western blot and immunohistochemical. The expression of caspase-9 was determined by Western blot. Results Compared with group S, NDS of group IR was significantly declined and the number of normal py- ramidal neurons in the area CA1 of the hippocampus was obviously decreased (P〈0. 05). After 24 hours of reperfusion the expression of XIAP was declined and the expression of Smac and caspase-9 was increased(P 〈0.05). Compared with group IR, NDS of group H was significantly increased and the normal pyramidal neurons in the area CA1 of the hippocampus was also significantly increased(P〈0. 05). The expreesion of XIAP was raised and the expression of Smac and caspase-9 was decreased (P〈0. 05).
