儿童型克拉霉素(利迈先)根除幽门螺杆菌的疗效观察

摘　　要：目的 :研究不同剂量儿童型克拉霉素对消化性溃疡患儿幽门螺杆菌根除和疼痛缓解的效果 ,以及不良反应的发生率。方法 :分别采用大剂量 (A组 )和小剂量 (B组 )克拉霉素 +质子泵抑制剂 (PPI) +甲硝唑三联疗法与羟氨苄青霉素 +PPI +甲硝唑三联疗法组 (C组 )对68例消化性溃疡患儿进行对比前瞻性研究。结果 :A组的HP根除率为96 % ,B组为91 3 % ,C组为90 % ,3组间无显著性差异 (P>0 05)。3组在治疗前、治疗后3天和治疗后1周的腹痛缓解程度均有显著性差异 (P<0 05) ,但3组间均无显著性差异 (P>0 05)。不良反应轻微 ,A组发生率为28 % ,B组为4 3 % ,C组为25 % ,B组与A组、C组间均有显著性差异 (P<0 05)。结论 :小剂量克拉霉素 +PPIOBJECTIVE:To observe the effects of different doses of clarithromycin-based triple therapy on eradication of Helicobacter pylori(HP),relieve of abdominal pain and incidence of adverse drug reactions in pediatric patients with peptic ulcer METHODS:A prospective study was made in 68 child patients with peptic ulcer receiving a triple therapy including high-dose(group A),low-dose(group B) clarithromycin or amoxicillin(group C)plus PPI and metronidazole RESULTS:Eradication rate of HP were 96%(24/25) in group A,91 3%(21/23) in group B and 90%(18/20) in group C No significance among them(P>0 05) There was significant difference of relieve of abdominal pain in every group before treatment,and in 3 days,1 week after treatment (P<0 05) But among three groups,there were no marked differences (P>0 05) The main adverse drug reactions were nause,vomiting,anorexia and mild diarrhoea,and special taste in a few cases Total incidence of adverse reaction was 28% in group A,4 3% in group B and 25% in group C There were significant differences among three groups(P<0 05) CONCL_USION:Clarithromycin in low dose+PPI+metronidazole is the best therapeutic scheme for eradicating HP in child patients
