摘　　要：目的探讨环指指动脉顺行岛状皮瓣与拇指桡侧逆行岛状筋膜皮瓣瓦合修复拇指缺损的临床应用效果。方法武汉科技大学附属普仁医院自2010年4月至2013年3月对10例Ⅰ类拇指缺损采用环指指动脉顺行岛状皮瓣与拇指桡侧逆行岛状筋膜皮瓣瓦合修复结合自体髂骨移植,治疗后观察皮瓣及骨愈合、皮瓣感觉功能及术后相关并发症发生情况。结果经过1年随访,10例拇指缺损修复良好,皮瓣及移植骨愈合良好,按中华医学会手外科学会拇、手指再造功能评定试用标准:优8例,良2例。无术后相关并发症发生。结论环指指动脉顺行岛状皮瓣与拇指桡侧逆行岛状筋膜皮瓣瓦合结合自体髂骨移植修复Ⅰ类拇指缺损的临床效果满意。Objective To evaluate the clinical effects of thumb reconstruction with digital artery flap of ring finger and radial retrograde fasicial flap of thumb. Methods Ten patients with type I thumb defects were chosen from our hospital since Apr. 2010 to Mar. 2013. All of them were treated by thumb reconstruction with digital artery flap of ring finger together with radial retrograde fasicial flap of thumb and combined crest bone graft. The outcomes of flap and bone healing, sensory function of flap and complications were observed. Results In the one-year follow-up, thumb defects in all patients were repaired. Flaps and grafted bones in all patients were healed. No evidence of post-operation complications was observed in the patients. The treatment in 8 patients showed excellent outcomes and well in 2 patients according to the assessment standard for finger reconstruction function by Hand Surgery Society of CMA. Conclusion Using the digital artery flap of ring finger and radial retrograde fasicial flap of thumb together with crest bone graft in the treatment of type I thumb defects reached a successful clinical outcome.
