摘 要:目的对比生物胶与缝扎法在肾创伤模型上的治疗效果。方法家兔32只,均分为两组,切除右肾后分别建立左肾切割伤和部分切除伤模型,各模型均分为生物胶组和缝扎法组(n=8),记录各组出血量、止血时间,术前及术后1、2、3、4周分别检测血肌酐水平,术后1周超声检查,术后1个月取创面行病理检查。结果肾切割伤生物胶组和缝扎组出血量分别是(3.09±0.56)m L和(5.77±1.33)m L,止血时间分别是(10.02±1.48)s和(66.40±6.35)s;肾部分切除伤两组出血量分别是(4.67±0.73)m L和(7.42±1.65)m L,止血时间分别是(16.16±3.51)s和(179.38±18.72)s,生物胶组与缝扎组的出血量及止血时间差异有统计学意义(P<0.05)。肾切割伤和肾部分切除伤中术后各时间段肌酐值生物胶组均明显低于缝扎组(P<0.05)。术后1个月病理检查提示生物胶组肾创面愈合较好,肾单位损伤明显轻于缝扎组。结论生物胶相对于缝扎法在肾创伤治疗中对肾组织的损伤和肾功能的影响更小,止血效果更好,是一种更安全有效的止血方法。Objective To compared the efficiency of biological glue and suturing in the treatment of rabbit model of renal trauma. Methods Thirty-two rabbits were inflicted with partial nephrectomy or excision wound in the left kidney after resection of the right kidney, and then randomly divided into 2 groups, the biological glue group and suture treated group (n = 8 for each damage model). Their bleeding amount, hemostatic time, and serum creatinine level before and in 1, 2, 3 and 4 weeks after surgery were recorded. Ultrasound imaging was performed on the injured left kidney in 1 week after surgery. Histological observation was carried out on the wound site of the kidney. Results For the rabbits of renal excision injury, the blood loss was 3.09 ±0. 56 and 5.77 ± 1.33 mL respectively for the animals treated by biological glue and by suturing, and the hemostatic time was 10.02 ±1.48 and 66.40 ± 6.35 s, respectively. For the rabbits suffering from partial nephrectomy, the blood loss was 4.67 ± 0.73 and 7.42 ± 1.65 mL, and the hemostatic time was 16. 16 ±3.51 and 179.38 ±18.72 s, respectively for the biological glue group and the suturing group. The blood loss was obviously less and hemostatic time was significantly shorter in the biological glue group than the suturing group (P 〈 0.05 ). Serum levels of creatinine were significantly lower at every time points in the biological glue group than the suturing group of no matter partial nephrectomy or excision wound models ( P 〈 0. 05 ). Pathological results showed biological glue group had better wound healing, and significantly mild renal unit damage than the suture group. Conclusion Biological glue is superior tosuturing in renal unit damage and renal function after renal trauma, and has better hemostatic effect. So it is safe and effective hemostatic method.
