摘　　要：目的建立改良的肿瘤截骨导板与相应的腓骨截骨导板模型,为下颌骨肿瘤患者的修复重建提供一种较为全面的手术预案。方法采用螺旋CT扫描仪扫描患者的下颌骨及供区腓骨,应用mimics 10.0、geomagic studio2013等软件模拟切割肿瘤、塑形移植腓骨,按照常规方法设计肿瘤截骨导板、腓骨截骨导板。在此基础上,模拟肿瘤软组织可能向前延伸10 mm的情况,设计改良的肿瘤截骨导板、腓骨截骨导板。结果改良的肿瘤截骨导板及腓骨截骨导板设计小巧,与骨组织贴合紧密,无需延长原手术方案切口。与传统方法比较,改良导板可兼顾原手术预案与术中出现的肿瘤边界大于"预期值"的情况。结论改良导板从理论上提高了手术精度、缩短了手术时间、完善了术后效果,具有较强的可行性,有待临床验证。Objective To establish modified timorous and fibutar cutting guiding plates in order to provide a comprehensive reconstruction plan for patients with mandibular tumor. Methods The mandible and fibula parts were scanned with the spiral CT, and the processes of cutting tumor and shaping fibula transplantation were simulated using mimics 10.0 and geomagic studio 2013 softwares, and the timorous and fibular cutting guiding plates were devised using the routine method. On the above basis, the condition that the tumor had exetended 10 mm anteriorly was simulated, and then modified timorous and fibular cutting guiding plates were devised accordingly. Results The modified timorous and fibular cutting guiding plates were small and tightly fit with bone tissues without extending the original surgical incision. Compared with that of routine method, the modified guiding plates could meet the conditions of primary surgical plan and the tumor boundary was larger than expected. Conclusion The modified timorous and fibular cutting guiding plates may enhance the operative accuracy, shorten the operative time and improve the surgical effect with good feasibility in theory, but it needs be further confirmation in clinical practice.
