新三联联合超声晶氧治疗胎儿窘迫的疗效分析

摘　　要：目的：探讨新三联联合超声晶氧治疗胎儿窘迫的疗效，为临床诊断治疗提供参考。方法将174例足月分娩活跃期出现胎儿窘迫的孕妇按照随机数字表法将其分为对照组和治疗组，每组各87例；对照组给予超声晶氧治疗，治疗组在对照组治疗基础上给予新三联治疗；比较两组胎心、羊水粪染改善情况， Apgar评分情况，胎儿出生脐动脉血气分析情况。结果治疗组30min内胎心恢复率（93．10％）显著高于对照组（72．41％），且有显著性差异（χ^2＝4．863，P＜0．05）；治疗组胎儿娩出时羊水无粪染率显著高于对照组，且有显著性差异（χ^2＝16．082，P＜0．05）；治疗组1min Apgar评分正常者显著高于对照组，且有显著性差异（χ^2＝13．931，P＜0．05）；治疗组胎儿出生脐动脉血气酸碱度（PH）、氧分压（PO2）、碱剩余（BE）均高于对照组，二氧化碳分压（PCO2）指标低于对照组，差异有统计学意义（t值分别为19．571、8．272、12．989、19．314，均P＜0．001）。结论新三联联合超声晶氧治疗胎儿窘迫疗效确切，能显著改善胎儿娩出时羊水粪染情况、1 min Apgar评分及出生脐动脉血气，具有重要的研究价值，值得在临床上推广使用。Objective To explore the clinical efficacy of new triple treatment combined with ultrasound crystal oxygen therapy on fetal distress, so as to provide reference for clinical diagnosis and treatment.Methods A total of 174 cases of full-term delivery with fetal distress at active stage were divided into control group and treatment group according to random number table with 87 cases in each group. The control group received ultrasound crystal oxygen therapy, while the treatment group was given new triple therapy additionally.Fetal heart rate, improvement of meconium-stained amniotic fluid, Apgar score and umbilical arterial blood gas analysis at birth were compared between two groups.Results The heart recovery rate within 30min in the treatment group （93.10%） was significantly higher than that in the control group （72.41%） （χ2 =4.863,P〈0.05）.The rate of amniotic fluid without meconium in the treatment group was significantly higher （χ2 =16.082, P ＆lt;0.05）.There were more cases with normal Apgar scores in the treatment group, and the difference was significant （χ^2 =13.931, P〈0.05）.Compared with the control group, cases in the treatment group had higher birth umbilical arterial blood gas PH, PO2 and BE but lower PCO2 index, and there were significant differences （ t value was 19.571, 8.272, 12.989 and 19.314, respectively, all P〈0.001）.Conclusion The new triple treatment combined with ultrasonic crystal oxygen treatment can obtain definite efficacy, improve meconium at birth and 1min Apgar score as well as umbilical arterial blood gas.It is of great importance and worthy of promotion in clinics.
