治疗糖尿病合并肺结核小量咯血对比观察

摘　　要：目的通过两组止血药物对糖尿病合并肺结核小量咯血治疗的对比观察,探讨不住院门诊治疗糖尿病合并肺结核小量咯血的可行性。方法经结核门诊一般检查或必要的特殊检查确认的糖尿病合并肺结核小量咯血病例,均采取措施控制血糖在正常范围,随机分为联合投药组(安络血和云南白药组)与单一投药组(云南白药组),进行止血效果临床观察,对两组的疗效和安全用药进行评价。结果安络血与云南白药联合投药组临床观察65例,5日止血有效率为78.46%;10日止血有效率为92.30%。云南白药单一投药组临床观察63例,5日止血有效率为61.90%,10日止血有效率为74.60%。两组临床数据采用四格表资料法进行χ2值显著性检验,5日止血有效率联合投药组优于单一投药组(P<0.05);10日止血有效率联合投药组优于单一投药组(P<0.01)。结论治疗糖尿病合并肺结核小量咯血两组比较,在控制血糖在正常范围的前提下,联合投药组5日止血有效率、10日止血有效率均优于单一投药组,即安络血与云南白药治疗糖尿病合并肺结核小量咯血疗效安全可靠,可以用于门诊治疗。Objective To compare hemostatic drugs were observed small amount of hemoptysis of pulmonary tuberculosis on diabetes treatment, and explore the feasibility of non-diabetic patients with tuberculosis treatment in hospital outpatient small amount of hemoptysis. Methods General inspection by tuberculosis clinic diabetes cases or small amount of hemoptysis of pulmonary tuberculosis necessary special inspection confirmed, are taking measures to control blood sugar in the normal range, they were randomly divided into combined administration group（security network of blood and Yunnan Baiyao Group） with a single dosage unit（Yunnan Baiyao group）, conducted hemostasis clinical observation of two groups to evaluate the efficacy and safety of medication. Results Yunnan Baiyao joint security network of blood and drug administration group,65 cases of clinical observation, the 5th bleeding rate was 78.46%; the 10 th bleeding rate was 92.30%. Yunnan Baiyao single administration group 63 cases of clinical observation, the 5th bleeding rate was 61.90 percent, the 10 th rate is 74.60% effective hemostasis. Comparison of clinical data using tables from method χ2 value significance test, the 5th hemostatic efficiency of the combined administration group than single administration group（P0.05）; 10 th hemostasis efficient joint administration group than the single dosage unit（P0.01）. Conclusion The small amount of hemoptysis of pulmonary tuberculosis treatment of diabetes between the two groups, in the control of blood glucose in the context of the normal range, the joint administration group on the 5th hemostatic effectiveness, the 10 th hemostatic effective rate than single administration group, namely security network of blood and Yunnan Baiyao small amount of hemoptysis of pulmonary tuberculosis treatment of diabetic efficacy safe, can be used for outpatient treatment.
