摘　　要：目的探讨医护一体化工作模式对泌尿造口患者自我感受负担的影响,为医护一体化模式不断规范与深入推广提供依据。方法选取2013年7月至2014年2月因膀胱癌于本科室实施泌尿造口患者74例作为对照组,2014年3月至10月因膀胱癌于本科室实施泌尿造口患者83例作为观察组,对照组实施传统医护合作照顾模式,观察组实施医护一体化服务模式,分别于入院时、术后1周及术后1月向两组发放Cousineau的自我感受负担量表(Self-Perceived Burden Scale,SPBS),并进行测量,比较两组自我感受负担的程度。结果两组入院时自我感受负担分值差异无统计学意义(P>0.05)。观察组患者术后一周及术后1月自我感受负担分值明显低于对照组,差异有统计学意义(P<0.05)。结论医护一体化工作模式,能明显降低造口患者自我感受负担,促进康复,提高患者生活质量。Objective The purpose of this study was to validate the influence of integrated health care model on the self-perceived burden of patients with urinary stoma, provide theoretical and empirical reference for nurses and finally to extend the utilization of this transitional care service.Methods 74 urinary stoma patients of the experimental group were selected from Hunan caner hospital from July 2013 to February 2014, and 83 cases urinary stoma patients of the control group were selected from March 2014 to October 2014. The patients of experimental group were treated in integrated health care model, and the control group was in medical cooperation model. They were both assessed on their burden experience by Self-Perceived Burden Scale （SPBS） at the time of admission, a week following surgery and a month after surgery.Re- suits There were no significant difference of SPBS in patients with urinary stoma at the time of admission between the experimental group and the control group （P〉0.05）. However, the SPBS score in the experimental group which benefit from integrated health care model was better than the control group, especially at the 1 week and 1 month after the surgery. （P〈0.05）.Conclusion Integrated health care model can obviously relieve the patients＇ experience of burden, promote rehabilitation and improve patient quality of life.
