摘　　要：目的探讨Wiltse入路(多裂肌与最长肌间隙入路)单侧椎弓根钉固定联合椎间融合治疗体重指数(BMI)超标患者腰椎间盘突出症的临床疗效。方法本组自2011-02-2014-08采用Wiltse入路(多裂肌与最长肌间隙入路)单侧椎弓根钉固定联合椎间融合治疗BMI超标患者腰椎间盘突出症83例,其中腰3-4椎间盘突出6例,腰4-5椎间盘突出40例;腰5骶1椎间盘突出37例。结果按Mac Nab评定标准:优76例,良6例,可1例,优良率98.8%。按Suk标准判断:已融合70例,可能融合6例,7例患者未复查。未发现一例未融合,无椎弓根钉及Cage松动、断裂及移位。结论 Wiltse入路单侧椎弓根钉固定椎间融合技术,手术方法简便,疗效确切,创伤小,出血少,并发症少,费用节省,易于推广,是治疗BMI超标患者腰椎间盘突出症的一种较好的选择。Objective To explore the Wihse approach （torn muscle and muscular clearance into the longest road） using unilateral nail stick fixed joint intervertebral fusion in the treatment of excess weight index combined the clinical curative effect of patients with lumbar disc pro- lapse. Methods From February 2011 to August 2014,our hospital used Wiltse approach （torn muscle and muscular clearance into the longest road） unilateral nail rod fixed intervertebral fu- sion method to treat excess weight index and the merger of 83 patients with lumbar disc pro- lapse,6 cases with lumbar intervertebral disc herniation 3-4,40 cases of lumbar 4-5 interverte- bral disc highlight, Waist 5 37 cases 1 intervertebral disc. Results By MacNab assessment standard： excellent in 76 cases,good in 6 cases,and fair in 1 case,the excellent and good rate was 98.8%. According to standard Suk：has merged in 70 cases,6 cases may fusion,7 patients without review. No fusion and pedicle screw looseness, breakage and cage shift were found. Con- cltmion The Wiltse approach using unilateral nail stick fixed intervertebral fusion technology has advantages of simple operation method, exact curative effect, small trauma, less bleeding, less cost, easy to promote,it is a better selection in the treatment of lumbar intervertebral disc protru- sion with excess weight index.
