珠海市结核病控制现状分析

摘　　要：目的 :通过分析珠海市 1995～ 2 0 0 0年结核病的流行病学特点、临床治疗情况 ,了解和掌握珠海市结核病控制的现状。方法 :回顾性的分析对比了项目内外结核病的治疗现状和流行病学特点。结果 :1)珠海市的结核病流行病学特点为肺结核占 96 .4 1% ,以男性为多 ,男女性别比为 1.82∶ 1(2 0 6 4 /1131) ;肺内、肺外结核均以 2 0～ 30岁年龄组登记病例最多 (39.0 3% ;19.33% ) ,30～ 4 0岁组次之 (2 1.34% ;8.4 % ) ;肺结核登记病例以农民最多 ,占 32 .18% ,工人占 31.6 4 %。2 )项目内、外肺结核病例的管治情况有很大差异 ,项目内能完成疗程者占 97.12 % (135 /139) ,项目外完成疗程者占 6 6 .0 3%(5 5 2 /836 ) ,明显低于项目内病例 ;按照全程督导管理项目病例 10 0 % ,而项目外病例只有 5 .6 2 % ,且病例的治疗方案多数不规范 (共有 35种 )。结论 :珠海市的结核病控制工作取得了明显成绩 ,但是项目外肺结核病人的管理和治疗仍较混乱。Objective:Find out and grasp the situation of tuberculosis control project in Zhuhai by analyzing the epidemiological characteristic and clinical therapeutic status.Methods:Compare the differences of tuberculosis therapeutic situation and the epidemiological characteristic between WB-loaned TB control project and non.Results:1)The characteristic of tuberculosis in Zhuhai is as follows:pulmonary thberculosis embraces 96 41 percent of all patients.Among those,the ratio of female and male is 1 82∶1,and mostly fall into the group whose age is between 20 and 30 years old,those whose age is between 30 and 40 years old take the second place.Furthermore,among the registered cases of tuberculosis,most of them are farmers and workers,make up of 32 18 percent and 31 64 percent respectively.2)There is a distinct difference of tuberculosis control between WB-loaned TB control project and non.The ratio of having finished therapy with and without the WB-loaned TB control project is 97 21 percent and 66 03 percent,respectively.The ratio of patients who are treated by DOTS with and without the project is 100 percent and 5 6 percent respectively.Moreover,there are 35 chemotherapy methods in non WB-loaned TB control project.Conclusion:There is more improvement in TB control in Zhuhai.Chemotherapy and management types are confusional in tuberculosis patients of non WB loaned TB control project.To stress on pulmonary tuberculosis patients classified management is necessary and efficient.The DOTS is very important.
