针灸治疗膝关节骨性关节炎选穴规律现代文献研究

摘　　要：目的：总结近20年临床文献中针灸治疗膝关节骨性关节炎的选穴规律。方法：检索1990年1月～2014年9月中国知网全文数据库（CNKI）和万方学术期刊全文数据库中有关针灸治疗膝关节骨性关节炎的文献，设计资料提取表，运用频数、百分数对文献中相关腧穴的归经、分布部位等特点进行统计描述。结果：纳入文献131篇，用穴58个。使用频数较高的腧穴依次为犊鼻（122）、内膝眼（1lO）、阳陵泉（108）、血海（100）、足三里（96）、阴陵泉（83）、梁丘（83）、鹤顶（59）、膝阳关（34）、三阴交（25）、委中（22）、曲泉（17）、悬钟（16）、关元（15）、太溪（14）、肾俞（13）。使用腧穴频数最高的经脉为足阳明胃经。腧穴分布最多的部位为下肢。结论：针灸治疗膝关节骨性关节炎选穴以循经取穴、局部取穴为主，部分结合辨证选穴，选穴相对集中。Objective：To summarize the rules of acupoint selection for acupuncture treatment of knee os- teoarthritis in the modern literatures of the rescent 20 years. Methods：The literatures in CNKI and Wanfang Data about acupuncture treatment of knee osteoarthritis from January 1990 to September 2014 were retrieved. The literatures database was established and the acupoints information including their channel tropism and locations were analyzed by frequency and percentage. Results：Total 58 points in t31 pieces of literature were included. Acupoints with high frequency included Dubi （ ST35,122 ） , Neixiyan （EX-LE4,110 ） , Yan- glingquan （GB34,108）, Xuehai （SP10,100）, Zusanli （ST36,96）, Yinlingquan （SP9,83）, Liangqiu （ST34,83）, Hed- ing （EX-LE2,59）, Xiyangguan （GB33,34）, Sanyinjiao （SP6,25）, Weizhong （BL40,22）, Ququan （LRS, 17 ）, Xu- anzhong （GB39,16）, Guanyuan （RN4,15）, Taixi （KI3,14）, Shenshu （BL23,13）. The majority of them are be- long to Stomach Meridian of Foot-Yangming. Most of them located on lower extremity. Conclusion ：The mod- ern acupuncture treatments of knee osteoarthritis focus on acupoints along meridians and local point selec- tion. Some of the points are selected according to syndrome differentiation.
