精简扶正解毒通络汤治疗慢性萎缩性胃炎癌前病变32例疗效观察

摘　　要：目的 :观察精简扶正解毒通络汤治疗慢性萎缩性胃炎癌前病变的临床疗效。方法 :62例慢性萎缩性胃炎伴癌前病变患者按随机数字表法分为治疗组32例和对照组30例。对照组口服胃复春片,治疗组给予精简扶正解毒通络汤,两组均以3个月为1个疗程,治疗2个疗程后观察其胃镜、病理和抗幽门螺杆菌疗效。结果:治疗组改善胃镜下黏膜充血、黏膜白相、颗粒增生、血管透见、胆汁返流的有效率分别为85.71%、64.29%、61.90%、65.00%、92.85%,对照组分别为60.00%、34.62%、27.78%、33.33%、54.55%,治疗组优于对照组,差异有统计学意义(P<0.05或P<0.01);治疗组病理学检查示改善腺体萎缩、肠上皮化生、异型增生的有效率分别为78.12%、82.61%、85.71%,对照组分别为53.33%、55.50%、45.45%,治疗组优于对照组,差异有统计学意义(P<0.05);治疗组幽门螺杆菌根除率54.54%,总有效率77.27%,对照组分别为47.36%和57.89%,差异无统计学意义(P>0.05)。结论 :精简扶正解毒通络汤治疗慢性萎缩性胃炎癌前病变具有较好的疗效。Objective：To observe the curative effects of Simplified Fuzheng Jiedu Tongluo decoction on pre- cancerous lesion of chronic atrophic gastritis（CAG）. Methods：Sixty-two cases of chronic atrophic gastritis with precancerous lesions were randomized into treatment group（rt=32） and control group（rt=30） by random num- ber table. The treatment group was given Simplified Fuzheng Jiedu Tongluo decoction and the control group was given Weifuehun tablets. Both groups were treated for three months as a course. After two courses,the gastroscopic and pathological examination and anti-Helicobacter Pylori （Hp） infection effect of the two groups were observed. Results：The effective rates of improvements of mucosal hyperemia,mucosal white-appearance, hyperplasia of mucosal particles,blood vessel witnesss,bile regurgitation under gastroscopy of the treatment group were 85.71% ,64.29% ,61.90% ,65.00% ,92.85% respectively,and those for the control group were 60.00% ,34~62% ,27.78% ,33.33% ,54.55% respectively,the differences of these items between the two groups were significant（P〈0.05 or P〈0.01）. The effective rates of improvements of gland atrophy,intestinal meta- plasia,dysplasia showed by pathological examination of the treatment group were 78.12%,82.＇61%,85.71% re- spectively,and those for the control group were 53.33% ,55.50% ,45.45% respectively,the difference of the above items between the two groups were significant（P〈0.05）. The eradication rate and total effective rate of Hp infection of the treatment group were 54.54% and 77.27% ,and those for the control group were 47.36% and 57.89%,the differences between the two groups were not significant （P〉0.05 ）. Conclusion：The Simplified Fuzheng Jiedu Tongluo decoction has good gastroscopic and pathological effects on precancerous lesions of CAG.
