摘 要:目的探讨膈神经在颈部的高频超声声像图特征。方法选择2011年8月至2012年10月健康成年志愿者40例,其中男性19例,女性21例;年龄19~54岁,平均年龄33岁。应用PHILIPS公司生产的i U 22型彩色多普勒超声诊断仪,线阵探头频率8~12 MHz,对健康成年志愿者膈神经进行检查。观察膈神经在颈部的位置、走形及与周围解剖结构的关系;测量膈神经前后径。结果 40例80条膈神经纵切呈条状低回声,边界呈高回声,横切成小圆点型低回声,其高回声外缘不明显。19例男性健康成年志愿者膈神经在颈部的前后径为(0.77±0.05)mm,高于21例女性健康成年志愿者[(0.60±0.03)mm],差异有统计学意义(t=18.60,P〈0.05);40条左侧膈神经在颈部的前后径为(0.69±0.10)mm,与40条右侧膈神经的[(0.68±0.10)mm]比较,差异无统计学意义(t=0.34,P〉0.05)。结论高频超声可以精确定位膈神经在颈部的位置,是膈神经较好的影像学检查方法。Objective To study the features of phrenic nerve in neck by high-frequency ultrasound image. Methods From August 2011 to October 2012, 40 healthy adult volunteers were enrolled, included 19 males and 21 females and aged 19 - 54 years old with mean age of 33 years old. All of them were performed ultrasound diagnosis with iU color Doppler ultrasonic diagnostic apparatus with probe frequency liner array of 8 - 12 MHz. The locations, shapes and surrounding structures of phrenic nerves in neck were observed, and anteroposterior diameters of phrenic nerve were measured. Results Eighty phrenic nerve of 40 cases showed strip slitting hypoechoic with hyperechoic at boundaries, and crossing of dot hypoechoic with inconspicuous hyperchoic edge. The average anteroposterior diameter of phrenic nerves in 19 healthy male volunteers was (0.77 ~ 0.05) ram, longer than that of 21 females volunteers[(0.60 ± 0.03) mml, and the difference was statistically signifieant(t = 18.60, P 〈 0.05). The average anteropostefior diameter of 40 left phrenic nerves was (0.69 ± 0.10) ram, while that of the fight phrenic nerves on was (0.68 ± 0.10) mm, and the difference was not statistically significant(t = 0.34, P 〉 0.05). Conclusion It is demonstrated that the high-frequency ultrasound could pinpoint the location of phrenic nerves in neck, which may provide a preferred imaging diagnostic method.
