CCR2和CCR5基因多态性与结核性脓胸的相关性研究

摘　　要：目的 探讨CCR2基因-190G/A和CCR5基因-59029G/A单核苷酸多态性（SNP）与结核性脓胸的相关性。方法 采用聚合酶链反应-限制性片段长度多态性（PCR-RFLP）方法对200例结核性脓胸患者和200例健康人进行-190G/A和-59029G/A SNPs检测,采用逐步logistic回归分析其与结核性脓胸的相关性。结果 与CCR2基因-190G/G基因型相比,携带A/G和A/A基因型均可增加结核性脓胸发病风险（OR=2.250,95%CI=1.046-4.842,P=0.037和OR=8.706,95%CI=4.232-17.909,P=0.000）。无卡介苗接种史亦增加结核性脓胸发病风险（OR=3.031,95%CI=2.109-4.356,P=0.000）。CCR5基因-59029G/A位点各基因型未进入回归方程,与结核性脓胸的发病风险无明显相关性。结论 CCR2基因-190G/A SNP与结核性脓胸有相关性,CCR5基因-59029G/A SNP可能与结核性脓胸无关。Objective To explore the relationship between tuberculous empyema and single nucleotide polymorphisms （SNPs） of CCR2 gene- 190G/A and CCR5 gene- 59029 G/A. Methods SNPs of CCR2 gene- 190G/A and CCR5 gene- 59029G/A were detected by polymerase chain reaction- restriction fragment length polymorphism （PCR- RFLP） in 200 pa- tients with tuberculous empyema and 200 healthy controls. And their correlations with tuberculous empyema were analyzed by stepwise logistic regression. Results Compared with the CCR2 gene - 190G/G genotype, carrying A/G（ OR = 2. 250, 95 % CI = 1. 046 - 4. 842, P = 0. 037） and A/A （ OR = 8. 706, 95 % CI = 4. 232 - 17. 909, P = 0. 000） genotypes might increase the risk of tuberculous empyema. No history of BCG vaccination could also increase the risk of tuberculous empyema （ OR = 3. 031, 95 % CI = 2. 109 - 4. 356, P = 0. 000）. CCR5 gene - 59029 G/A locus genotypes did not enter the regression equation, indicating that Jt had no significant correlation with the risk of tuberculous empyema. Conelusions CCR2 gene - 190G/A SNP has a correlation with tuberculous empyema, while CCR5 gene - 59029 G/A SNP may be unrelated to it.
