激光联合微波术在肛门尖锐湿疣疾病中的治疗效果分析

摘　　要：目的：观察并比较单纯使用CO2激光、He-Ne激光联合微波术、CO2激光联合微波术治疗肛门尖锐湿疣（CA）患者的临床疗效,探讨临床最佳治疗方案。方法：选取2011年9月至2014年11月期间来我院治疗的178例肛门尖锐湿疣患者作为研究对象。根据自身情况和医生建议,患者选择合适的治疗方案,其中单纯使用CO2激光治疗患者为56人,设为对照组。选择CO2激光联合微波术进行治疗的患者有70人,而另外52位患者选择了He-Ne激光联合微波术进行治疗。分别评估三组患者治疗后创面愈合时间、疼痛指数、治愈率、完全治愈所需次数并进行比较,统计学分析。结果：He-Ne激光联合微波术和CO2激光联合微波术治疗组患者创面愈合所需时间分别为7~9d和7~10d,所需时间少于对照组所需愈合天数10~15d。这三组患者在治疗当天均有不同程度的疼痛,治疗1周、2周、3周之后,He-Ne激光联合微波术组患者疼痛比例分别为44.23%、34.62%、9.62%;CO2激光联合微波术治疗组患者疼痛比例分别为45.71%、32.86%、8.57%;单纯CO2激光治疗组患者疼痛比例分别为62.5%、39.29%、17.86%。这3种治疗方案均出现患者复发现象,治疗3个月之后,对照组、He-Ne激光联合微波术和CO2激光联合微波术复发率分别为37.50%、13.36%、12.86%。对复发患者进行再治疗,对照组所需次数要多于治疗组（He-Ne激光联合微波术或CO2激光联合微波术）。结论：激光联合微波治疗肛周尖锐湿疣疗效较好,是治疗肛周尖锐湿疣较好的手段。Objectives： Investigating a better therapy for curing patients with condyloma acuminate （CA） through comparing the effect of CO2 laser, microwaves combined with CO2 laser, and He - Ne laser. Methods： 178patients diagnosed with CA from Sep. 2011 to Nov. 2014 in our hospital were selected and divided into different groups according to their therapy ： 56 cases of CO2 laser treatment were regarded as control group; 70 patients choo- sing microwaves combined with CO2 laser and another 52 patients choosing therapy of microwaves combined with He - Ne laser were selected for research. The pain index/cure rate and time used to treat patients in the three groups were recorded and analyzed. Results： The time required for wound healing of microwave combined with He - Ne la- ser and microwave combined with COz laser were 7 - 9 days and 7 - 10 days respectively, which were fewer than that of control group （ 10 - 15 days）. Before treatment, patients in the 3 groups had varying degrees of pain. After treatment for 1 week, 2 weeks, 3 weeks, the proportions of patients with pain in microwave combined with He - Ne laser group were 44.23%, 34.62% and 9.62%, respectively; in microwave combined with CO2 laser group, the proportions were 45.71%, 32.86% and 8.57% respectively; in CO2 laser group, the proportions were 62.5%, 39.29% and 17.86% respectively. After 3 months of treatment, recurrence rates in control group, microwave com- bined with He - Ne laser group and microwave combined with CO2 laser group were 37.50%, 13.36%, and 12.86% respectively. Retreatment was performed, where control group required more times than treatment group （microwave combined with He - Ne laser or microwave combined with CO2 ） for total cure. Conclusion： Micro- waves combined with laser （ He - Ne or CO2 laser） is better in curing patients with CA.
