摘 要:目的对一种自助听力测评系统进行初步临床评估,为其在大范围人群推广应用提供实验依据。方法95例志愿者采用自助听力测评系统在噪声小于45dB(A)的非隔声室环境下自主进行250、500、1000、2000、4000Hzl司值的测试,然后由专业人员采用GSI-61纯音听力计(在本底噪声低于30dB(A)的标准隔声室中)进行相同频率阈值的测试,计算各频率闽值差值。根据两种方法测得的平均听闽分别对所有受试者进行听力障碍分级,应用Kappa检验进行一致性分析。计算系统诊断中度及以上听力障碍的灵敏度和特异度。结果自助听力测评系统与纯音听力计测得的阈值吻合度较好。Kappa检验显示根据两种方法进行听力障碍分级的结果具有很好的一致性(Kappa=0.858,P=0.00)。系统诊断中度及以上听力损失的灵敏度为100%,特异度为98.9%。结论本自助听力测评系统提供了一种快速便捷的自助式听力测评方法,经对比试验验证测得的结果较为准确,但需进一步在较大规模人群中进行应用验证。Objectivo To conduct a preliminary clinical evaluation for an automated hearing test system in order to provide experimental evidences for its application to large-scale population. Methods A total of 95 volunteers performed a hearing test with the automated hearing test system by themselves in a quiet room with background noise less than 45 dB(A) and received pure-tone audiometry with GSI-61 clinical audiometer by audiologists in a soundproof room with background noise less than 30 dB(A). The hearing thresholds at 250,500, 1000, 2000 and 4000 Hz were obtained and the differences between the two methods were compared. The degree of hearing impairment of each volunteer was assessed according to the results of the two methods respectively and the Kappa test was applied to analyze the consistency of the results. The sensitivity and specificity of the system in detecting the moderate and more severe hearing impairment were calculated. Results The consistency of hearing thresholds between automated hearing test system and pure-tone audiometer was good. Kappa test showed that there was a good consistency in the hearing rating results between the two methods(Kappa=0.858,P=0.00).The sensitively of the system in detecting the moderate and more severe hearing impairment was 100% and the specificity was 98.9%. Conclusion This system provides a quick and convenient automated hearing test method, and can obtain a relatively accurate result. However, the system should be further assessed in large-scale population.
