摘 要:肝脏纤维化进展和恶性病变发生时病灶组织肝动脉和门静脉血流量会发生明显改变,测量肝组织血流量对于检测肝脏组织病变进展和监测治疗效果具有重要的价值。PET/CT是将PET与CT有机结合在一起进行解剖、功能和分子成像的先进成像设备。采用PET/CT和正电子核素示踪剂能够无创、精准获得肝脏组织总血流量,以及肝动脉、门静脉所占的血流量比例。特别是采用动脉输入图像派生法替代传统采集动脉血样的方法后,使得测量肝脏血流量的方法更加简便,因此该方法广泛应用于临床前期研究和临床研究。就无创PET/CT分子成像设备测量肝脏总血流量以及肝动脉、门静脉血流量的方法和应用进展予以介绍。Measuring hepatic tissue blood flow is of great value for the detection of liver tissue lesion progression and monitoring the therapeutic efficacy as hepatic artery and portal venous blood flow vary significantly in the process of liver fibrosis or malignant progression. PET/CT is the most advanced anatomical, functional and molecular imaging equipment that integrates superiorities of PET and CT. We can obtain hepatic artery, portal venous blood flow, and the ratio of each with the help of PET/CT accurately and noninvasively. Particularly the methods for liver blood flow measurement become much simple with the use of image-derived arterial input function, instead of arterial blood sample, hence has been widely used in preclinical and clinical research. This paper mainly introduces the methods for measuring total blood flow, hepatic artery, and portal venous blood flow by using noninvasive PET/CT.
