跟骨解剖板在波及跟距关节的跟骨骨折中应用探讨

摘　　要：目的探讨应用跟骨解剖板治疗波及跟距关节的跟骨骨折的临床价值。方法自2010年7月—2012年12月,应用跟骨解剖板治疗波及跟距关节的跟骨骨折患者28例,术中注意关节面、Bohler角和Gissane角的复位,同时采用跟骨解剖板及松质骨螺钉进行牢固固定。结果 28例患者平均随诊23个月,患者主观症状满意率82.1%,足部外形优92.9%,足及踝关节活动优67.9%,Bohler角优85.7%。结论跟骨解剖板治疗波及跟距关节的跟骨骨折,固定牢固,允许术后早期功能锻炼,能最大限度减少骨折后的并发症,显著提高患者的生活质量。Objective to evaluate the clinical value of applying calcaneus anatomy plate to treat intra-articular fractures of calcaneus.Methods From July 2010 to December 2012,we applied calcaneus anatomy plates to threat 28 cases of crumbled fracture of calcaneus. During operation , we paid attention to the reduction of articular face ,Bohler angle and critical angle ,and made secure internal fixation with calcaneus anatomy plate and cancellous bone screw. Results 28 cases were followed up for 23 months averagely. 82.1%of the patients were satisfactory with the subjective symptoms ,while the rate of excellent foot outline was 92.9%. The rate of excellent joint motion of foot and ankle was 67.9%, and the rate of excellent Bohler angle was 85.7%. The overall excellent and good rate was 92.6%.Conclusion Calcaneus anatomy plate on intra-articular fractures of calcaneus can acquire secure internal fixation ,and permit patient to exercise earlier after operation ,and can greatly decrease complications of calcaneus fracture and enhance the patients＇ quality of live.
