宫腔镜电切除子宫内膜息肉的疗效及安全性分析

摘　　要：目的探讨宫腔镜下子宫内膜息肉切除术的疗效及安全性。方法选取2012年7月—2014年7月我院收治的确诊为子宫内膜息肉并行宫腔镜下子宫内膜息肉电切术的患者40例,记录患者术中情况;收集和对比患者术前术后阴道排液、阴道分泌物、阴道异常出血、血红蛋白、月经天数、月经量等情况。结果本组患者手术成功率为100%,平均手术时间为(17.1±4.3)min,平均手术出血量为(31.2±10.1)m L,平均膨宫液用量为(953.4±287.3)m L。1例(1.0%)患者术后出现感染,经抗生素治疗2 d后体温恢复正常;其余患者未出现手术后子宫异常出血、子宫腔粘连、子宫周围脏器损伤等手术并发症。与手术前相比,手术后阴道排液异常,阴道异常分泌物、子宫异常出血、不孕减少,血红蛋白升高,月经天数缩短,月经量评分降低,差异有统计学意义(P<0.05)。结论宫腔镜下子宫内膜息肉切除术疗效佳、安全性高,可广泛应用于临床。ObjectiveTo explore the curative effect and safety of hysteroscopic endometrial polyps resection. Methods40 patients diagnosed to be endometrial polyp undergoing hysteroscopic endometrial polyps resection from July 2012 to July 2014 in our hospital were selected and intraoperative conditions of them were recorded. Vaginal discharge,leucorrhea,abnormal vaginal bleeding,infertility,Hb,menses days,menstrual blood volume of patients were collected and compared before and after the operation.ResultsAll patients underwent hysteroscopic endometrial polyps resection and there were no patients without operation failed and success rate was 100%. The average operation time was（17.1±4.3）min,the average bleeding volume during operation was（31.2±10.1）ml, the average distention fluid volume was（953.4±287.3）ml. A patient（1.0%）had infection after operation and returned to normal 2 days after antibiotic therapy. Other patients did not have operation complication such as endometrorrhagia,adhesion of uterine cavity,uterine injury to surrounding organs. Compared to patients before operation,patients after operation had less abnormal vaginal discharge,abnormal leucorrhea,abnormal vaginal bleeding,higher Hb,shorter menses day,lower menstrual quantity score and the difference was statistically significant（P〈0.05）.ConclusionCurative effect and safety of hysteroscopic endometrial polyps resection is better and it can be widely used in clinical application.
