摘　　要：目的 比较不同年龄患者前交叉韧带重建手术的效果.方法 选取2010年10月至2012年12月因前交叉韧带断裂行前交叉韧带重建手术的患者57例进行回顾性分析.年龄＞40岁患者28例,男13例,女15例;平均年龄46.5（41 -63）岁;年龄＜25岁患者29例,男19例,女10例,平均年龄21.0（18 -24）岁.平均随访时间26.0（24 - 45）个月.所有患者术前和随访时均行Lysholm、Tegner评分和国际膝关节文献委员会膝关节评估表（IKDC）评分以及膝关节动度仪（KT-2000）测量,对结果进行统计学处理并分析.结果 年龄＞40岁和年龄＜25岁的患者术后平均Lysholm、Tegner和IKDC评分分别为89（74 -100）、5（3 -6）、90 （70 -100）和94（79 -100）、5（4-7）、93（74- 100）分,均较术前显著改善（P＜0.05）,两组差异无统计学意义（P＞0.05）.两组患者术后屈膝30.和90.KT-2000差值均较术前明显减小,平均差值分别为2（0-4）、1（-1 -4）和2（0-4）、1（0-4） mm,两组差异无统计学意义.结论 ＞40岁与＜25岁患者前交叉韧带重建手术的临床效果相似.Objective To compare the outcomes of anterior cruciate ligament （ACL） reconstruction in patients aged over 40 years and under 25 years at least 24 months follow-up after operation.Methods Fifty-seven patients who underwent ACL reconstruction for ACL rupture in Institute of Sports Medicine, Peking University Third Hospital,from October 2010 to December 2012 were selected for this retrospective study.The patients aged over 40 years consisted of 13 men and 15 women with an average age of 46.5 years （range, 41 to 63 years）.The patients younger than 25 years consisted of 19 men and 10 women with an average age of 21.0 years （range, 18 to 24 years）.The average follow-up time was 26.0 months （range, 24 to 45 months）.Clinical functional evaluation for all patients was performed according to Lysholm, Tegner and IKDC scores both before and after surgery.In addition, a pre-and post-operative arthrometric evaluation was also performed with KT-2000.Results The average postoperative Lysholm, Tegner and IKDC scores in patients aged over 40 years and under 25 years were 89 （74 to 100）, 5 （3 to 6）, 90 （70 to 100） and 94 （79 to 100）, 5 （4 to 7）, 93 （74 to 100） respectively.The average postoperative KT-2000 arthrometer anterior laxity in 30° and 90° in both groups was 2 mm （0 to 4 mm）, 1 mm （-1 to 4 mm） and 2 mm （0 to 4 mm）, 1 mm （0 to 4 mm） respectively.The postoperative functional scores and side-to-side difference improved more significantly than preoperative values, with no significant difference in both groups.Conclusion ACL reconstruction provides the comparable clinical outcomes between middle-aged patients and young patients
