摘  要:目的本文着重研究"小病进社区、大病进医院、康复回社区"模式下的转诊制度实施情况,并就存在的问题提出建议。方法现场调查绍兴市人民医院实施双向转诊以来的就诊患者,收集已有的相关资料,主要通过描述性分析方法定量分析双向转诊情况。结果双向转诊率低,只有0.18%;但建立医联体的医院与社区之间的双向转诊人数占了总数的92%;双向转诊制度的知晓率只有6%。结论分级诊疗刚刚起步,双向转诊任重道远,工作开展需要着力点。Objective This paper focuses on the implementation situation of the referral system under the mode of “go into the community for minor illness, go to the hospital for serious illness and return to the community for rehabilitation”, and puts forward suggestions for the existing problems. Methods The patients who received medical services in Shaoxing People's Hospital since the implementation of two-way referral were surveyed, and the existing relevant materials were collected to analyze the situation of two-way referral with descriptive analysis. Results The two-way referral rate was low, only 0.18%, but the number of people who were referred between hospitals and communities in the health alliance accounted for 92% of the total. The awareness rate of two-way referral is only 6%. Conclusion Grading diagnosis and treatment were iust launched. The two-wav referral is a long and arduous task, and the focus of the work should be identified.
