不同术式治疗混合痔术后肛门动力学改变情况比较

摘　　要：目的探讨两种不同手术方式治疗混合痔后对肛门动力学的影响情况,以为临床治疗提供参考依据。方法选取自2013年2月至2014年2月于我院普外科就诊的混合痔患者122例,随机均分为试验组和对照组,试验组接受TST手术治疗,对照组接受PPH手术治疗;对比观察两组患者入院时、术后3月和半年肛门动力学变化情况。结果两组患者手术情况无差异,术后并发症对照组高于试验组;试验组术后3个月和术后半年直肠感觉阈均与术前有差异(P<0.05),其他均无明显差异且恢复到正常参考值范围内;对照组术后3个月和术后半年,直肠静息压、肛管静息压、直肠肛管压力差均较术前有明显改变。结论 TST术式更适用于维持混合痔患者术后肛门动力学,值得临床推广应用。Objective To investigate the anal dynamics changes under different surgical treatment of mixed hemorrhoids,and then elect the better one. Methods 122 cases of patients with mixed hemorrhoids were collected,randomly divided into experimental and control groups,the former received TST,and the latter received PPH; the change of anal dynamics between the two groups were compared preoperative,three months and six months after surgery. Results There was no difference in surgery cases,but postoperative complications in control group was higher than the test group; in the experimental group,the rectal sensation threshold at three months and six months after surgery were different with the preoperative（ P〈0. 05）,there were no other significant differences and returned to within the normal reference range; in the control group the rectal resting pressure,anal resting pressure,anorectal pressure difference were significant changes than before surgery at three months and six months after surgery. Conclusions The TST surgical was more suitable to maintain anal dynamics for mixed hemorrhoid patients,worthy of clinical application.
