臭氧联合激光治疗下肢静脉曲张性溃疡的临床对照研究

摘　　要：目的比较臭氧联合激光静脉内消融(OEVLT)治疗下肢静脉曲张性溃疡相对于单纯激光静脉内消融(EVLT)的临床疗效。方法 92例静脉曲张性溃疡患者被随机分为OEVLT和EVLT两组,在臭氧联合激光静脉内消融组中,患者在接受激光静脉内消融前先接受臭氧治疗,所有患者最短随访12个月。结果在12个月后的随访中,两组之间的治疗静脉的闭塞率无明显区别,在OEVLT组,有患者在治疗后的1周溃疡就愈合。OEVLT与EVLT相比,溃疡的愈合率明显提高,且患者的满意度明显提高,溃疡的复发率明显下降。两组均没有发生严重的并发症及副反应。结论与单纯EVLT相比,OEVLT治疗静脉曲张性溃疡具有更好的疗效,更少的复发率。OEVLT治疗静脉曲张性溃疡是一种很好的方法,值得推广应用。Objective The aim of this study was to evaluate the clinical efficacy of ozone gas bath combined with EVLT in comparison with EVLT alone for the treatment for lower limb venous ulcers. Methods Ninety- two patients with venous ulcers were randomized to receive ozone gas bath combined with EVLT（ OEVLT group） or EVLT alone（ EVLT group）. In the OEVLT group,the venous ulcers were preconditioned with ozone gas bath prior to EVLT. The minimum follow- up time was twelve months. Results There was no significant difference in venous occlusion between the two groups. Ulcer healing occurred as early as 1 week after the procedure in the OEVLT group. The rate of ulcer healing in the OEVLT group was significantly higher than the EVLT group within 12 months follow- up. Patients in the OEVLT group showed better satisfaction and less recur rate than the OEVLT group. No severe complications or side effects occurred in either group. Conclusions Ozone gas bath combined with EVLT showed improved efficacy for the treatment of lower limb venous ulcers and less recur in comparison with EVLT alone. This procedure is a safe and technically feasible.
