炎琥宁配合持续气道正压通气治疗先天性心脏病合并重症肺炎患儿的临床意义

摘　　要：目的:探讨持续气道正压通气结合炎琥宁治疗先天性心脏病合并重症肺炎患儿的临床意义。方法:先天性心脏病合并重症肺炎患儿60例根据随机抽签原则分为治疗组与对照组各30例。对照组给予常规治疗措施及持续气道正压通气治疗,治疗组在对照组的基础上给予炎琥宁进行治疗。结果:治疗后1h后经过观察,治疗组有效率显著高于对照组(P<0.05)。治疗前两组PaO2和SaO2对比无明显差异(P>0.05);在治疗1h、6h后,两组的PaO2和SaO2显著升高(P<0.05),不过治疗组与对照组相比,PaO2和SaO2升高更加明显,组间对比差异明显(P<0.05)。治疗前,两组血清BNP含量无显著差异,治疗后均明显上升(P<0.05),同时治疗组的血清BNP含量明显高于对照组(P<0.05)。结论:持续气道正压通气结合炎琥宁治疗先天性心脏病合并重症肺炎患儿可使患儿的动脉血氧饱和度和血氧分压迅速而显著提高,临床症状明显改善,其作用的发挥与降低血清BNP表达有关。Objective：To investigate the clinical significance of continuous positive airway pressure combined Yanhuning injection for treating congenital heart disease with severe pneumonia in children.Methods：60congenital heart disease with severe pneumonia children based on the principle of random draw were equally divided into the treatment group and the control group,the control group were received conventional treatment and continuous positive airway pressure,the treatment group were added the Yanhuning injection treatment.Results：After treatment 1hthrough observation,the total efficiency of the treatment group and the control group were 90.0% and 60.0%respectively that compared were significantly difference（P〈0.05）.The PaO2 and SaO2values compared between the two groups before treatment had no significant difference（P〉0.05）;After treatment 1h,6h,the PaO2 and SaO2values in the both groups were significantly higher（P〈0.05）,but the treatment group compared with the control group,PaO2 and SaO2 were increased more obvious than the control group（P〈0.05）.The serum BNP levels comparison between the two groups were not statistically significant before treatment,after treatment,the serum BNP levels were showed a clear upward trend（P〈0.05）,while serum BNP levels in the treatment group were significantly higher（P〈0.05）.Conclusion：CPAP intermittent positive pressure ventilation combined Yanhuning injection in the treatment of severe pneumonia in children with congenital heart disease can quickly improve arterial oxygen saturation and blood oxygen and improve clinical symptoms,it plays its role expressions may be related to lower serum BNP expression.
