开环式微创痔上黏膜切除吻合术治疗混合痔的临床研究

摘　　要：为探讨开环式微创痔上黏膜切除吻合术（TST）治疗混合痔的临床疗效及安全性，将70例混合痔患者随机分为观察组和对照组，各35例，观察组行TST治疗，对照组行PPH治疗，对比两组患者手术情况及术中、术后并发症。结果显示，两组患者均治愈（治愈率100％）。两组手术时间、住院时间及术后肛缘水肿、尿潴留、原发性出血、肛门狭窄发生率比较，差异均无统计学意义，P〉0．05。但在术中牵拉反射及术后急便感、肛门疼痛程度方面，观察组均明显优于对照组，P〈0．05。结果表明，TST治疗混合痔疗效肯定，与PPH相比，患者术后痛苦更小，并发症更少，值得临床推广应用。To explore the clinical effect and safety of open loop-shaped TST for mini-invasively treating mixed hemorrhoids,the 70 cases of mixed hemorrhoids were randomly divided into observation group and control group, n = 35 for each;the former received above procedure, while the latter received PPH;then, the intraoperation status and postoperative complication were compared between both groups.As results, the curative rate in both groups was all 100%,in operating time, hospitalization duration, and postoperative incidences of anal edge edema, urosthesis, primary bleeding as well anal stenosis there was no statistical difference between both groups（ P〉0.05）, but, in intraoperative pull reflex,postoperative fecal urgency and pain degree of anus the former was all significantly superior to the latter （ P〈0.05）.Results show that the above procedure have certain effect, but compared with PPH, the above procedure is of more milder pain,and less complication,thus is worthy to be popularized to use clinically.
