中药对肝胃不和型功能性消化不良症状及内脏敏感性影响的随机对照研究

摘　　要：[目的]观察和胃冲剂治疗功能性消化不良(FD)肝胃不和证的临床疗效。[方法]选取符合西医FD及中医肝胃不和证的受试者80例,运用计算机随机法分为试验组(41例)和对照组(39例),分别给予和胃冲剂及吗丁啉口服,疗程均为4周。比较治疗前后2组症状积分及内脏敏感性的变化。[结果]治疗后2组症状积分均较治疗前改善,且试验组优于对照组(P<0.05);在上腹痛等单项症状积分改善上试验组优于对照组(P<0.05);试验组总有效率为95.12%,对照组为76.92%,2组比较差异有统计学意义(P<0.05)。水负荷联合B超试验,治疗后阈值饮水量(TV)和饱足饮水量(SV)试验组的改善程度优于对照组(P<0.05),但阈值近端胃容积(SPTV)和饱足近端胃容积(SPSV)治疗后2组间差异无统计学意义。[结论]和胃冲剂临床总有效率达95.12%,通过调节内脏敏感性,改善FD患者的消化不良症状,尤其在上腹痛、早饱、嗳气、烧心这4个单项症状改善上优于吗丁啉。[Objective]To observe the clinical effect of Weihe granules in treating functional dyspepsia （FD）of liver-qi depression syndrome. [Methods]We enrolled 80 subjects suffering from both FD and dis- harmony between liver and stomach syndrome. The subjects were randomly divided into the experimental group（41 cases）and the control group（39 cases）. The subjects in the experimental group took Weihe gran- ules for 4 weeks,while those in the control group took domperidone. We observed the changes of symptom scores and visceral sensitivity of the two groups before and after treatment. [Results]Symptom scores of the two groups were improved after treatment, and the experimental group was better than the control group （P〈0.05）. The experimental group was superior to the control group in the four symptom scores,namely epigastric pain, early satiety,belching and heartburn（P〈0.05）. The total effective rate of the experimental group（95.12%）was statistically higher than that of the control group（76.92%）,P〈0. 05. The water load test combined with ultrasound indicated that the scale of improvement of threshold volume（TV） and satie- ty volume（SV） after treatment in the experimental group was superior to that in the control group（P〈 0.05） ,whereas there was no statistical difference in threshold proximal stomach volume（SPTV） and satie-ty proximal stomach volume（SPSV） between the two groups.[Conclusion]The clinical total effective rate of Weihe granules can reach 95.12%. It can regulate visceral sensitivity to relieve clinical symptoms of pa- tients suffering from FD,which is superior to domperidone, especially in the symptoms of epigastric pain, early satiety, belching and heartburn.
